摘要
目的观察川芎嗪注射液对严重脓毒症心肌损伤的治疗效果及其作用机制。方法将符合纳入标准的42例严重脓毒症心肌损伤患者随机分为治疗组和对照组,每组21例。对照组按照《脓毒症指南》给予抗生素、液体复苏、血管活性药物、器官功能支持治疗;治疗组在对照组基础上给予川芎嗪注射液静滴(80mg+G S/NS250 mL,ivd,qd×7d)。于纳入的第1,3,7天检测患者的血清肌钙蛋白、脑钠肽、丙二醛、超氧化物歧化酶(SO D)含量。结果治疗组血清肌钙蛋白、丙二醛下降较对照组快,而治疗后2组患者的SOD均有所升高,以治疗组升高较显著,2组间比较差异均有统计学意义(P<0.05)。结论川芎嗪注射液有减轻严重脓毒症患者心肌损伤的作用,其机制与川芎嗪抗氧化反应有关。
Objective To explore and discuss the effect of tetramethylpyrazine(TMP) on myocardial injury by sepsis and explore its possible mechanism.Methods 42 patients with myocardial damage caused by sepsis were randomized divided into treatment group(n=21) and control group(n=21).All patients were treated with antibiotic,liquid revive,vasoactive drugs and organ support by conventional treatment programs recommended by international sepsis treating guidelines.Patients of treatment group were also given intravenous TMP(80 mg + GS/NS250ml,ivd,qd ×7 days),on the basis of conventional treatmet.Indexes of CTnT,BNP,MDA and SOD were tested and recorded on the first day,third day and seventh day.Results CTnI and MDA levels in treatment group dropped more dramatically than that in control group,and SOD levels in treatment group increased more dramatically than that in control group the difference was statistically significant(P0.05).Conclusion TMP can improve myocardial injury of patients with sepsis.Its mechanism may be related to the fact that TMP can reduce oxidative stress.
