泌尿系结石患者中段尿细菌培养及药敏分析 被引量:12

Analysis of mid-stream urine culture and its drug sensitivity in urinary calculi

摘要 目的了解铜陵地区近3年泌尿系结石患者中段尿病原菌感染分布及其药物敏感情况,为围手术期合理选择抗菌药,防治术后感染提供有效参考。方法对2008年1月至2010年8月199例泌尿系结石患者中段尿细菌培养和药敏结果进行回顾性分析研究。结果 199例患者中37例(占18.6%)中段尿培养阳性,致病菌38株,1例为复合菌感染,其中手术患者尿培养阳性24例(占12.1%)。所有尿培养阳性中革兰阴性菌30株占78.9%,以大肠埃希菌21株(55.3%)最常见;革兰阳性菌占18.5%,以粪肠球菌(10.5%)最常见;真菌占2.6%。产超广谱β-内酰胺酶(ESBLs)大肠埃希菌对头孢类和喹诺酮类敏感性很低,对哌拉西林/他唑巴坦、亚胺培南敏感性较高,敏感率分别为70%,100%。结论大肠埃希菌仍是泌尿系结石患者尿液感染的主要致病菌,产ESBLs菌近年来呈增加趋势。泌尿系结石患者尤其需行手术者应先行中段尿培养和药敏试验,合理选择抗菌药,有效防治术后感染。 Objective To analyze the pathogenic bacterial's distribution and the drug sensitivity in patients with urinary calculi in Tongling area and to provide the information for choosing suitable antibacterial in perioperative therapy.Methods From January 2008 to August 2010,the result of mid-stream urine bacterial culture and its drug sensitivity test in 199 patients with urinary calculi were analyzed retrospectively.Results Pathogens presented in 37(18.6%) patients with 38 bacteria strains.There were 30(78.9%) Gram-negative bacteria strains.Escherichia coli were the predominant bacteria,accounted for 55.3%(21 strains).There were 10(18.5%) Gram-positive bacteria strains,the predominant bacteria was Enterococcus faecalis(4 strains),accounting for 10.5%.There was 1 fungi strain(2.6%).ESBLs-producing Escherichia coli strains were susceptible to piperacillin/tazobactam and imipenem,but resistant to most cephalosporins and quinolones.The rate of sensitivity to piperacillin/tazobactam and imipenem were 70 and 100.Conclusion Escherichia coli are the most common uropathogen causing urinary infection in patients with urinary calculi.The detection rate of ESBLs-producing Escherichia coli kept raising yearly.Mid-stream urine bacterial culture and its drug sensitivity test should be taken to choose suitable antibacterial before treatment especially for perioperative patients with urinary calculi.
