^(18)F-FDGPET/CT在诊断鼻咽癌复发及远处转移中的价值 被引量:3

摘要 目的比较MRI与18F-FDG PET/CT在鼻咽癌放疗后局部复发与纤维化鉴别诊断中的应用价值并对18F-FDG PET/CT显像鼻咽癌患者远处转移的诊断效能进行分析。方法 41例鼻咽癌患者经放疗后,均行18F-FDG PET/CT扫描,其中27例行MRI平扫及GD-DTPA增强检查,以病理和临床随访结果作为最后诊断,分析鼻咽癌远处转移部位、发生率,PET/CT采用5级分类诊断法,选取不同分级点作为截止点绘制ROC曲线,寻找最佳截止点评价诊断效能。结果18F-FDG PET/CT较MRI对鼻咽癌放射治疗后局部复发与纤维化的鉴别诊断有更重要的应用价值,复发灶与纤维化SUV比较有统计学意义(P<0.01)。41例远处转移发生率为63%,最常见的部位是远处淋巴结、骨骼。ROC曲线示以3级为最佳截止点PET/CT诊断效能较高,敏感性92.3%,特异性73.3%。结论18F-FDG PET/CT较MRI对鼻咽癌放射治疗后局部复发与纤维化的鉴别诊断有更重要的应用价值,PET/CT诊断鼻咽癌远处转移的效能较高。
