摘要
目的探讨经脐孔单次穿刺行腹腔化疗联合静脉化疗的有效性及安全性。方法对49例卵巢癌术后患者采用经脐孔单次穿刺法行腹腔化疗联合静脉化疗。治疗方案:紫杉醇135 mg/m2(d1,静脉给药);顺铂60 mg/m2(d2,腹腔给药),每3周重复一次。结果 49例患者共腹腔化疗160疗程,总穿刺成功率98.75%(158/160),其中一次穿刺成功率95.63%(153/160),二次穿刺成功率3.12%(5/160)。穿刺失败率1.25%(2/160);全组总并发症发生率4.38%(7/160),未发生感染、肠穿孔、粘连性肠梗阻等严重并发症。随访12~64个月,1年生存率97.96%(48/49),2年生存率93.88%(46/49),3年生存率89.80%(44/49),中位无疾病进展时间25个月。结论卵巢癌术后一线腹腔化疗联合静脉化疗可明显延长患者的无疾病进展生存期。经脐孔单次穿刺法行腹腔化疗操作简单、安全、成功率高,并发症发生率低,值得临床推广应用。
Objective To investigate the clinical efficacy and safety of intraperitoneal chemotherapy combined with intravenous chemotherapy. Methods 49 patients with ovarian cancer after surgery were treated with intraperitoneal chemotherapy combined with intravenous chemotherapy (the recommended chemotherapy regimen is paclitaxel, 135 mg/m^2 iv day 1, plus cisplatin 60 mg/m^2 intraperitoneal, day 2, repeat every 3 weeks). Results 49 cases of ovarian cancer underwent 160 times of postoperative intraperitoneal chemotherapy. The to tal puncture success rate was 98.75% (158/160) , among which the first and secondary puncture success rate was 95.63% (153/160) and 3.12% (5/160) ,respectively. Failure rate 1.25% (2/160) ;The total complica- tion rate was 4.38% (7/160). There were no infection, intestinal perforation, adhesive intestinal obstruction and other serious eomplieations. The patients were given follow-up from 12 to 64 months. The median PFS was 25 months respectively. One-year survival rate was 97.96% (48/49) , two-year survival rate was 93.88 % (46/ 49 ), three-year survival rate was 89.80% (44/49). Conclusion Intraperitoneal chemotherapy, combined with intravenous chemotherapy can prolong PFS obviously in postoperative ovarian cancer. Intraperitoneal chemotherapy through single puncture through the umbilicus is simple and safe, with high success rate and low complication rate, which is worthy of clinical application.
