摘要
目的分析小骨窗开颅血肿清除术和颅骨钻孔血肿穿刺外引流术治疗高血压脑出血患者的临床疗效。方法将我院2010年1月至2011年1月期间收治的54例高血压脑出血的患者随机分为观察组和对照组各27例,观察组采用小骨窗开颅血肿清除术治疗,对照组采用颅骨钻孔血肿穿刺外引流术治疗,比较两种治疗方法的临床疗效。结果观察组手术时间(1.03±0.35)h,术中出血(50.75±5.04)mL,住院时间(11.05±1.05)d,对照组手术时间(2.81±1.01)h,术中出血(186.25±11.74)mL,住院时间(21.40±5.18)d,两组比较差异有统计学意义(P<0.05);观察组优11例,良7例;对照组优4例,良9例,两组比较差异有统计学意义(z=-2.113,P<0.05)。结论小骨窗开颅血肿清除术治疗高血压脑出血创伤小、住院时间短,可降低患者术后再出血率,且促进患者神经功能恢复,值得临床推广应用。
Objective To explore the efficacy of small skull-window craniotomy versus that of cranial burr hole hematoma nyxis external drainage for hypertensive intraeerebral hemorrhage. Methods 54 patients with hypertension-induced cerebral hemorrhage in our hospital from January 2010 to January 2011 were randomly assigned to receive small skull-window craniotomy （27 patients, control group ） , and cranial burr hole hematoma nyxis external drainage （ 27 patients, observation group ）. The clinical effects were compared between the two groups. Results The surgical duration, bleeding volume,length of hospital stay were better in the observation group with the corresponding data of（ 1.03 ± 0.35 ） h, （50.75 ± 5.04 ） mL, and （ 11.05 ± 1. 05 ） d than in the control group with the data of（2.81 ± 1.01 ） h, （ 186.25 ±11.74） mL, and （21.40 ± 5.18 ） d, （ P 〈 0.05 ）. There were 11 eases of excellent,7, cases of good in the observation group and there were 4 cases of excellent,9 eases of good in the control group, which were significant differences between the two groups （z = - 2.113, P 〈 0.05 ）. Conclusion Small skull-window craniotomy has shorter surgical duration and length of hospital stay, less intraoperative bleeding, can reduce postoperative bleeding rate and promote the recovery of neurological function ,which makes it worthy of clinical application.
