弥散加权成像在新生儿早期缺氧缺血性脑病诊断中的应用

摘要 [目的]探讨磁共振弥散加权成像(DWI)技术在新生儿缺氧缺血性脑病(HIE)诊断中的应用价值.[方法]通过对120例HIE病人进行磁共振成像(MRI)常规及DWI检查,比较两种MRI成像技术诊断HIE的敏感率,并测量病变与正常组织表观弥散系数(ADC)值的差异.[结果]MRI常规成像诊断HIE 88例,DWI诊断108例,敏感率分别为70%、90%,两种检查技术敏感率相比较差异有显著性(P〈0.05);受损组织中心与正常组织的ADC值相比较差异有显著性(P〈0.01).[结论]DWI可以明显提高早期HIE的诊断检出率;ADC值指标能量化组织受损程度,对HIE临床诊断及治疗有着重要的应用价值. [Objective]To explore the value of diffusion-weighted imaging(DWI) in the diagnosis of neonatal hypoxicischemic encephalopathy(HIE). [Methods] Totally 120 HIE patients were examined by conventional MRI and DWl. The sensitive rate of two MR imaging methods for the diagnosis of HIE was compared. The difference in apparent diffusion coefficient(ADC) value between lesions and normal tissues were measured. [Results] The 88 HIE patients were diagnosed by conventional MRI and 108 HIE patients were diagnosed by DWI, and the sensitive rate was 70% and 90% , respectively. There was significant difference in the sensitive rate between two detection techniques( P 〈0.05). There was significant difference in ADC value between the center of damage tissues and normal tissues ( P 〈 0.01 ). [Conclusion] DWI can obviously improve the diagnostic rate of early HIE. ADC value can quantify the degree of damage tissues, so it has the important value in the clinical diagnosis and treatment of HIE.
