卵巢肿瘤三维能量多普勒血管指数与病理微血管密度的相关性研究 被引量:4

Research of correlation between the vessels index detected by three-dimensional color power angiography and pathological microvessel density in ovarian tumors

摘要 目的探讨采用三维能量多普勒超声(3D-CPA)检测进行卵巢肿瘤的血管分型及定量分析血管指数(VI)与术后标本病理微血管密度(MVD)之间的关系。方法采用3D-CPA检测116例卵巢实性或囊实性肿块的VI并进行血管分型,术后应用抗CD34单克隆抗体对病理切片进行免疫组织化学染色,计测肿瘤MVD,并进行VI与MVD相关性分析。结果Ⅰ~Ⅳ型血流的VI及MVD值逐级增加,Ⅲ型及Ⅳ型血流的VI及MVD明显高于Ⅰ~Ⅱ型,恶性组的VI及MVD明显高于良性组(P<0.05)。VI与MVD之间呈正相关(r=0.83,P<0.01)。以Ⅵ≥0.04条/cm^3诊断卵巢恶性肿瘤的敏感度为90.3%,特异度为82.2%。结论 3D-CPA计测VI值及血管分型与MVD具有密切相关性,能很好地反映卵巢肿瘤血管生成状况。3D-CPA是鉴别卵巢肿瘤良恶性的有效方法。 Objective To investigate the correlation between the type of vessels and quantitative analysis vessels index (VI) detected by 3D-CPA and microvessel density (MVD) by pathological specimens after operation in ovarian tumour. Methods The vessels index(VI) and the type of vessels were detected by 3D-CPA in 116 patients who had ovarian tumour. After operation the pathological specimens were examined by immunohistochemical staining to detect MVD by applicating single anti-CD34 clone antibodies. The relevant analysis were made to definite the corre- lation between VI and pathological MVD. Results The VI and MVD of I - IV type vessels increased gradually. The VI and MVD of III and IV type vessels were significantly higher than those of I - II type vessels, and the VI and MVD in malignant tumour groups were significantly highter thanthose in benign tumour groups( P 〈 0. 05 ). There was positive correlation between VI and MVD (r =0. 83 ,P 〈0. 01 ). When the VI≥0. 04/cm3 served as diagnostic criteria of ovarian malignant tumour,sensitivity and specificity were 90. 3% ,82. 2% respectively. Conclusion The vessels index(VI) and the type of vessels detected by 3D-CPA were closely correlated with MVD. 3D-CPA can well refleet generating conditions of the ovarian turnout vessels. It was an effective way to identify benign or malignant of ovarian tumour.
