摘要
目的应用中医及西医两种疗效评价方法评价肺瘤平膏联合化疗治疗中晚期非小细胞肺癌的疗效。方法采用前瞻性随机对照的方法共观察中晚期非小细胞肺癌患者119例,分为两组,其中单纯化疗组56例、化疗+肺瘤平组63例。两组采用国际通用的化疗方案治疗,化疗+肺瘤平组同时合并服用肺瘤平膏,两组疗程均42天。以包含临床症状、瘤体、Karnofsky(KPS)评分、体重、免疫功能评价的中医治疗中晚期肺癌患者临床受益(疗效)评定标准和WHO实体瘤疗效评价标准同步进行疗效观察。结果按照WHO实体瘤疗效评价标准评价,两组疗效差异无统计学意义(P>0.05);按照中医治疗中晚期肺癌患者临床受益(疗效)评定标准评价,化疗+肺瘤平组疗效优于单纯化疗组(P<0.05)。化疗+肺瘤平组在改善临床症状、免疫功能降低方面优于单纯化疗组(P<0.05)。化疗+肺瘤平能够改善化疗引起的白细胞、粒细胞降低、减少肺部症状(P<0.05),并能显著改善便秘症状(P<0.01)。两组在KPS评分、体重、中位生存期和1年生存率方面比较差异无统计学意义(P>0.05)。结论中医治疗中晚期肺癌患者临床受益(疗效)评定标准与WHO实体瘤疗效评价标准相比更能反映中医治疗肿瘤的特色与优势,益气养阴活血解毒中药肺瘤平膏在治疗中晚期非小细胞肺癌方面具有增效减毒作用。
Objective To compare the characteristic and differenc e between TCM and Western Medical therapeutic evaluation methodologies for the combination treatme nt with Feiliuping Extract（extract for treating lung cancer） and chemotherapy i n advanced nonsmall-cell cancer（NSCLC）.Methods Using prospective random-control method,totally 119 patients with advan ced NSCLC were divided into the chemotherapy group and combined therapy group.Chemotherapy was administrated to 56 patients in the chemotherapy group,and F eiliuping Extract was added to 63 patients in the combined group.The course of treatment lasted for 42 days.The curative effect was observed by evaluating the symptoms,tumor body,Karnofsky score,weight and immune function.Results Acco rding to Standard for Therapeutic Effect Evaluation of Solid Tumor by WHO,the t herapeutic effect was no significantly different between groups（P0.05）.A ccord ing to Standard for Therapeutic Effect Evaluation of TCM treatment for Advanced Lung Cancer,the therapeutic effect of the combined group was superior to that o f the chemotherapy group（P0.05）.Comparing with the chemotherapy group,F eiliu ping Extract could relieve symptoms especially pulmonary symptoms,promote the i mmune function,increase the count of WBC and granulocyte and improve constipati on condition efficiently（P0.05 or P0.01）.There was no significant difference between groups in Karnofsky score,weight,middle life span and survival rate o f one year（P0.05）.Conclusion Comparing with Sta ndard for Therapeutic Effect E valuation of Solid Tumor by WHO,Standard for Therapeutic Effect Evaluation of T CM treatment for Advanced Lung Cancer could better reflect the characteristics a nd advantages of TCM treatment for tumor.Feiliuping Extract is effective for ad vanced NSCLC by supplementing qi,nourishing yin,activating blood and eliminati ng toxin with its synergy and attenuation.
