摘要
目的 :探讨影响原发性先天性青光眼术后视力的原因及预防措施。方法 :追踪检查 5 1只接受手术治疗后 10年以上的原发性先天性青光眼的眼压、视力、角膜及杯盘比值等的变化情况。结果 :手术不成功的 12只眼视力全部在 0 .0 5以下。手术成功的 39只眼视力在 0 .1至 0 .8以上 ,7只单眼发病者视力都在 0 .2以下 ;角膜直径大于 13mm的 2 7只眼中 2 1只眼有 Haab线存在 ;杯盘比值大于 0 .2以上者 12只眼。结论 :手术不成功 ,长期高眼压是造成原发性先天性青光眼盲目的根本原因 ;手术成功后视力的好坏则取决于术前视神经受损害 。
Purpose:To detect the causes affecting postoperative visual acuity of primary congenital glaucoma and preventive measures.Methods:To follow up and examine the changes of intraocular pressure,visual acuity,corneal and C/D of 51 primary congenital glaucoma eyes who had been performed operation for more than ten year.Results:the visual acuity were all less than 0 05 of 12 failure operating eyes,ranged from 0 1 to 0 8 or better of 39 successful operating eyes and less than 0 2 of 7 single illed eyes;21 eyes had haab striae of 27 eyes whose corneal diameter were more than 13 mm;there were 12 eyes whoes C/D were larger than 0 2.Conclusions:Long term intraocular hypertension following failure operation was the basic cause resulting in blindness of primary congenital glaucoma;the visual acuity after successful operation depended on the preoperative damage of optic nerve,the degree of corneal damage and occurrence of postoperative amblyopia.
