摘要
目的调查新疆维吾尔自治区某县级医院住院患者费用变化情况，探讨不同付费方式对住院费用的影响因素，为医院费用管理提供参考。方法采集该县级医院2006年1月至2008年12月住院患者20740例的住院费用相关信息，分析3年间住院费用构成情况及因不同付费方式所产生的住院费用差别。结果03zo740例住院患者中，男9515例，占总人数45．9％，女11225例，占总人数翻．1％。（~006至2008年次均平均住院总费用为1971．15元，其中床位费、西药费、诊疗费、化验费、护理费、检查费分别占次均住院总费用的8．6％、34．7％、11．0％、9．6％、1．9％和6．2％。③2006、2007和2008年的人均住院费用分别为1952．30、1909．70和2052．10元。④不同付费类别中，自费与新型农村合作医疗方式比较，2006年的差异有统计学意义（1785．90元比2350．20元，P〈0．05），2007和2008年的差异无统计学意义（1639．80元比1881．30，1887．40比1771．30，P〉0．05），除此以外，不同付费方式的两两间比较的差异均有统计学意义（P〈0．05）。结论2006年至2008年住院费用构成中，次均费用从高到低排前三位的是西药费、诊疗费和化验费；最低为护理费。不同付费类别的次均住院费用存在差异，主要是由其费用构成不同所致。
Obojective To explore the influential factors of hospitalization expenses at one county hospital in Xinjiang province from 2006 to 2008. Methods To collect inpatients cases from a county hospital of Xinjiang to investigate the influencing factors of hospitalization expenses. Results Among 20740 in-patients, there were 9515 male（45.9% ） and 11225 female（54.1% ）. The average cost of hospitalization from 2006 to 2008 was 1971.15 RMB. The bed cost, medicine cost , treatment cost , assay cost, nursing cost and checking cost were 8.6%, 34. 7%, 11.0%, 9.6%, 1.9% and 6.2% of total cost respectively. The average cost of hospitalization for one patient in 2006,2007 and 2008 was respectively 1952.30, 1909.70 and 2052.10 RMB. The difference of the personally liable for costs and the new rural cooperative medical way in 2006 was statistically significant（ 1785.90 RMB vs 2350. 20 RMB,P 〈 0.05）. Conclusion The hospitalization expenses increase rapidly. The average hospital expense of different payment mode is different because the composition of cost is different.
