摘要
目的探讨老年院内难免压疮与院外带入压疮的危险营养因素的差异,提出有效防治措施。方法采集某院消化科年龄≥60岁的144例院内难免压疮(院内组)与77例院外带入压疮(院外组)老年患者的相关临床营养指标。用χ2检验,Logistic回归方法分析比较影响两组患者压疮发生的危险营养因素及差异。结果院外组老年压疮患者合并肿瘤、卧床时间≥20天、理想体重百分数<90%、小腿围<30 cm及未给予针对性营养支持者显著多于院内组(P<0.05);院内组老年压疮患者因疾病导致食欲减退、饮食类型改变及贫血者显著多于院外组(P<0.05)。Logistic回归分析结果:卧床时间长、小腿围围值减少、未给予针对性营养支持和饮食类型改变分别是导致院外组和院内组压疮发生的重要营养危险因素。结论 "人群针对性"的营养评估,应作为营养干预的基础列入压疮管理;建立出院老年卧床患者护理回访服务模式,有助于降低老年压疮发生率。
Objective To explore the differences of the risk nutrient factors between pressure ulcer acquired inevitably in hospital or brought outside the hospital in elderly patients,and provide effective interventions.Methods The clinical nutrition indexes of 144 erlderly patients（≥60 years old） with inevitable pressure ulcer acquired in hospital（nosocomial group） and 77 patients with pressure ulcer brought outside the hospital（outside group） were collected.The risky nutrient factors and differences in the two groups were investigated using χ2 test and logistic regression analysis.Results Cases with tumor,≥20 days of bed time,90% ideal body weight percentage,30 cm calf circumference and without targeted nutrition support in the outside group were more than in the nosocomial group（P0.05）;Cases with anorexia and changes of diet type caused by illness and anemia in the nosocomial group were more than in the outside group（P0.05）.The important nutrition risk factors for the occurrence of pressure ulcer in the outside group included the long bed time,decreased calf circumference and no specific nutrition support,in the nosocomial group included changes in diet type.Conclusion The crowd targeted nutrition evaluation should be put into pressure ulcer management as the foundation of nutrition intervention.Establishment of a nursing feedback mode for elderly bedrid patients discharge from the hospital helps to reduce the incidence of pressure ulcers.
