摘要
目的选择原发性高血压患者为研究对象,探讨血脂代谢异常对其血压变异性(BPV)的影响。方法入选231例40~65岁的中老年原发性高血压患者(均为初次确诊原发性高血压或经询问病史已停用所有降压药物>2周)。其中单纯高血压组119例(A组),其中男69例,女50例;高血压合并脂代谢异常组112例(B组,3个月内均未服用任何调脂药物),其中男66例,女46例。两组均询问饮酒史、吸烟史,记录年龄、性别、身高、体质量、病程、诊所血压(CBP),并计算体质量指数。测量空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)。对入选的患者进行24h动态血压监测。收集BPV长时变异指标,将两组进行对比分析,了解血脂异常对高血压患者BPV的影响。结果与单纯高血压组比较,高血压合并血脂异常组BPV增大,差异有统计学意义(P<0.01)。结论高血压合并血脂异常组较单纯高血压组BPV增大。提示高血压和血脂异常均是心脑血管疾病的重要危险因素,两者之间存在密切的关系,两者联合干预治疗对减少脑血管疾病的发生、发展、预后具有重要的意义。
Objective To select the patients with essential hypertension as the research object, discuss abnormal lipid metabolism on blood pressure variability (BPV) effect, Methods Selected age in the ,40 -65 years old in elderly patients with essential hypertension, in which 231 patients were randomly selected (were initially diagnosed with essential hypertension or through history has been deactivated all antihypertensive drugs 〉2 weeks). Simple hypertension group with 119 patients (group A), the group of 69 male cases, female 50 cases; hypertension complicated with abnormal lipid metabolism in group of 112 patients (group B), (both three months without taking any lipid regulating drugs), including 66 male cases, female 46 cases. The two groups were asked about drinking history, history of smoking, age, gender, height records, weight, duration of disease, clinic blood pressure (CBP), and calculated body mass index. Routine biochemical measurements of fasting plasma glucose (FPG), total cholesterol (TC), triglyccride (TG), low density lipoprotein cholesterol (LDL- C), high density lipo- protein cholesterol (HDL - C). For selected patients with 24h ambulatory blood pressure monitoring. Collection of BPV long time variability, after the two groups were compared and analyzed, understanding of lipid abnormalities in hypertensive patients with BPV effect. Results Compared with uncomplicated hypertension, hypertension combined with blood lipid abnormality group BPV increases, difference has obvious statistical significance (P 〈 0. 01 ). Conclusion Hypertension combined with blood lipid abnormality group than the pure hypertension group BPV increases. Tips for hypertension and dyslipidemia are impor- tant risk factors for cardiovascular and cerebrovascular diseases, the two are closely related, both combined intervention on reducing the occurrence of cerebrovascular diseases, development, has important prognostic significance.
