电视辅助胸腔镜和右胸小切口心脏手术及术后生活质量的比较

Quality of life and outcome of video-assisted thorascopic heart surgery and mini-right thoracic incision heart surgery:A comparative study

摘要 目的:比较电视辅助胸腔镜(VAT)和右胸小切口(MRTI)心脏手术的并发症及远期生活质量。方法:从2009年1月~2009年12月,从住院患者中选取10例诊断为房间隔缺损、室间隔缺损和单纯二尖瓣病变的患者,随机分成2组:VAT组(n=5)和MRTI组(n=5),比较术后近远期并发症及远期生活质量。结果:VAT组患者术后并发症及远期生活质量远优于MRTI组:VAT带气管插管时间[(3.8±0.8)h]显著小于MRTI组[(11.4±6.1)h,P<0.05];VAT组ICU时间[(14±5)h]显著小于MRTI组[(22±4)h,P<0.05];VAT组术后住院时间[(5.4±1.1)d]显著小于MRTI组[(8.6±1.1)d,P<0.01];VAT组术后1年的生活质量SF-36评分(123±6)显著大于MRTI组(109±4,P<0.01)。结论:对于适当选取的单纯房间隔缺损、室间隔缺损和单纯二尖瓣病变的患者VAT手术优于MRTI心脏手术。 AIM: To compare the complications and long-term quality of life of heart surgery by video- assisted thoraseopy and mini-right thoracic incision. METHODS: From January 2009 to December 2009, ten patients diagnosed as having atrial septal defect or ventricular septal defect or simple mitral lesion were assigned by double-blind method to two groups: video-assisted thorascope (VAT) group (n = 5) and mini-right thoracic incision (MRTI) group ( n = 5 ). Complications and long-term quality of life post- operatively were compared between groups. RESULTS: Complications in the VAT group were signifi- cantly less than those in the MRTI group. Quality of life of the VAT group was significantly better than that in the MRTI group. CONCLUSION: Video-assisted thorascopic heart operation is the most optimal approach for patients with atrial septal defect or ventrieular septal defect or simple mitral lesion.
