摘要
目的:应用超声心动图评价原发性高血压患者心脏构型及功能的变化。方法:对笔者所在医院治疗的53例原发性高血压患者进行超声心动图评价,并设对照组,为正常人员23例。测量升主动脉根部内径(AAO)、左心房前后径(LA)、室间隔厚度(IVS)、左室后壁厚度(LVPW)、左室舒张末期内径(LVEDD)、射血分数(EF)、左室舒张早期二尖瓣峰值血流速度(E)和舒张晚期二尖瓣峰值血流速度(A)及E/A值,组间均数比较行方差分析。结果:与正常对照组相比Ⅰ组升主动脉内径(AAO)及室间隔厚度(IVS)明显增加(P<0.05),Ⅱ组左心房前后径(LA)、室间隔厚度(IVS)、左心室后壁厚度(LVPW)与正常对照组及Ⅰ组比较差异均有统计学意义(P<0.05),与正常对照组相比Ⅰ组、Ⅱ组二尖瓣血流E峰、A峰、E/A差异均有统计学意义(P<0.05),三组间射血分数(EF)比较差异无统计学意义(P>0.05)。结论:原发性高血压患者心脏构型及心室功能均有改变,超声心动图成像技术可以准确评价原发性高血压所至的不同程度的心脏构型及功能变化。
Objective:To investigate the changes of cardiac geometry and ventricular function in Essential hypertensive patient by using Echocardiography.Methods:53 patients with Essential Hypertention and 23 age matched healthy subjects as normal control group were included.Echocardiographic parameters included:arise aorta (AAO),left atrial (LA),anterior-posterior diameter,thickness of interventricular septum (IVS) and left ventricle posterior wall(LVPW),LV end-diastolic diameter (LVEDD),ejection fraction (EF),early diastolic peak velocity(E) and atrial contraction peak velocity(A) of the mitral valve.E/A were also calculated.Results:Compared with normal control group,Ⅰgroup had higher AAO and IVST(P〈0.05).LA,IVS,LVPW in II group were larger than those in normal control group and groupⅠ(P〈0.05).Compared with normal control group,the difference of E,A,E/A in group Ⅰand Ⅱ were significant (P〈0.05).There was no significant difference among these three groups in EF(P〈0.05).Conclusion:In essential hypertensive patient,the patterns of cardic geometry and ventricular function were changed,Echocardiography could evaluate these various changes accurately.
