2型糖尿病一级亲属血清脂联素、胰岛素抵抗与大血管病变的研究 被引量:2

Relationship between serum adiponectin,insulin resistance and macrov ascular disease in nondiabetic first-degree ralative of type 2 diabetic mellitus

摘要 目的:探讨2型糖尿病(T2DM)患者正常糖耐量一级亲属血清脂联素、胰岛素抵抗(IR)与大血管病变的关系。方法:根据颈总动脉血管超声,将T2DM患者正常糖耐量一级亲属分为斑块组及非斑块组,同时选择年龄、性别匹配的无糖尿病家族史的健康人作为正常对照组,进行血压、空腹血糖(FPG)、血脂、血清脂联素以及胰岛素(FINs)的测定。结果:T2DM患者一级亲属斑块组和非斑块组高密度脂蛋白胆固醇(HDL-C)、血清脂联素水平明显低于正常对照组,收缩压、FPG、TG、胰岛素低抗指数(HOMA-IR)明显高于正常对照组;斑块组血清脂联素水平低于非斑块组。血清脂联素水平与HDL-C呈正相关,与血压、TG、FINs、HOMA-IR呈负相关。结论:血清脂联素可能在T2DM一级亲属胰岛素抵抗及大血管病变的发生发展中有重要作用。 Objective:To explore the relationship between serum adiponectin,insulin resistance and macrovascular disease in nondiabetic first-degree re(a)lative of type 2 diabetic mellitus(NFDR).Methods :Blood pressure,fasting plasma glucose(FPG),lipids,fasting serum insulin(FINs) and asiponectin were detected in NFDR with plaque and without plaque and health control subjects.Results :The levels of HDL-C and adiponectin in NFDR with plaque and without plaque were significantly lower than those in health control group,while blood pressure,FPG,TG,HOMA-IR in NFDR with plaque and without plaque were significantly higher than those in health control group.The level of adiponectin in NFDR with plaque group was lower than that in NFDR without plaque group.Adiponectin was correlated positively with HDL-C,and correlated negatively with blood pressure,TG,FINs,HOMA-IR.Conclusion:The levels of serum adiponectin may play an important role in the development of insulin resistance and macrovascular disease in nondiabetic first-degree relatives of type 2 diabete mellitus.
