瞬态诱发耳声发射频域分析的临床意义

摘要 目的 :确定瞬态诱发耳声发射 ( TEOAE)频域分析的应用价值。方法 :对正常耳、噪声暴露耳、梅尼埃病耳、中耳病变耳及对侧抑制耳 ,进行 TEOAE频域测试。结果 :1噪声暴露耳 ,一些频率带的频带信噪比降低 ,主频出现移动变化 ;2梅尼埃病耳 ,甘油摄入后 ,频带信噪比增高 ,未引出 TEOAE的部分耳出现 TEOAE,主频也有移动变化 ;3中耳病变耳 ,不同的鼓室病变耳的频带信噪比降低 ,程度各异 ,其变化与声导纳改变有关。另外 ,鼓膜虽有轻微变化 ,但声导抗值正常之耳 ,也低于正常。 4对侧抑制耳 ,1k Hz及 2 k Hz的频带信噪比降低。结论 :TEOAE测试的频域分析 ,是一种敏感参数 ,且有一定的频率特异性 。 Objective:To determine the clinical value of analysis of frequency domain of transient evoked otoacoustic emissions (TEOAE).Method:Frequency domain of TEOAE were measured in ①normal functional ears;②noise exposure ears;③Meniere′s ears;④pathologic ears of media ear;⑤contralateral suppression of normal ears.Result:①After exposure to noise,the value of band signal/noise ratio were decreased in certain frequconcy band,main frequency of FFT response were changed in frequency spectrum.②In the ears of Meniere′s disease,after the administraton of glycerol,the value of band signal/noise ratio were increasd in certain frequency band,no TEOAE were elicited before the adminitration of glycerol,but they appeared after glycerol administraction in some ears ,main frequency were changd in frequency spectrum,③In the pathologic ears of media ears.The value of band signal/noise ratio decreased to different extent with different kind of patlolggic changes,which is related to the change of acoustic immittance.Besides.Acoustic immittance were normal in those with mild pathologic change in the tympanic menbrane.The value were decreased in band signal/noise ratio.④After contralateral suppression of normal ears,1 kHz and 2 kHz band signal/noise ratio value were decreased.Conclusion:Frequency domain of TEOAE is a sensitive parameter and has property of frequency specificity.It is of great help to study the fanction of outer hair cells farther and make a differential diagnose in the clinial practice.
