摘要
目的:研究小剂量布托啡诺对芬太尼诱发咳嗽反应的影响。方法:选择择期手术患者180例,ASAⅠ-Ⅱ级,年龄18-50岁,体重45-80kg,随机分为四组,每组45例。外周静脉Ⅰ组(SS组)给予生理盐水10ml,外周静脉Ⅱ组(SB组)给予布托啡诺10μg/kg,给药途径均为外周静脉;中心静脉Ⅰ组(CS组)给予生理盐水10ml,中心静脉Ⅱ组(CB组)给予布托啡诺10μg/kg,给药途径均为中心静脉。所有患者在5min后全麻诱导时均与之前同样途径给予芬太尼5μg/kg,2s内注完。观察注完芬太尼后1min内咳嗽发生例数,记录第一次给药后3min(T1)和气管插管后1分钟(T2)患者的收缩压(SP)、舒张压(DP)、心率(HR)和脉搏血氧饱和度(SpO2)。结果:SS组、SB组、和T2时的收缩压(SP)、舒张压(DP)和心率(HR)变化无统计学意义(P>0.05)。结论:10μg/kg布托啡诺能够显著抑制芬太尼诱发的咳嗽反应。
Objective:To observe the effects of small dose of butorphanol on fentanyl-induced cough.Methods:One hundred and eighty ASA I and Ⅱ patients between the ages of 18 to 50 yr,weights of 45 to 80kg,undergoing selective surgery,were allocated randomly into four groups with 45 cases each.Patients in group SS were injected with 10ml saline through peripheral venous and the same in group CS through central venous before anesthesia.Patients in group SB and group CB were received butorphanol 10μg/kg via peripheral venous and central venous respectively.All of them were given fentanyl 5μg/kg within 2 seconds 5 min later by the same route.The coughes occurred in the next 1 min after fentanyl injection were recorded.The SP,DP,HR and SpO2 were also measured at 3 min after the butorphanol injection(T1)and 1 min after tracheal intubation(T2).Results:26(57.78%),2(4.44%),43(95.56%) and 7 patients(15.56%) had cough in group SS,SB,CS and CB respectively(P0.05).There were no significantly difference in SP,DP and HR among the four groups at T1 and T2(P0.05).Conclusion:10μg/kg butorphanol can alleviate fentanyl-induced cough effectively.
