摘要
目的探讨不同类型胃黏膜肠腺化生类型中Ki67、p53、HER-2、Bcl-2的表达及其临床意义。方法采用免疫组织化学染色方法检测2001年1月至2010年9月华山医院及宝山分院萎缩性胃炎肠腺化生的377例组织标本石蜡块Ki67、p53、HER-2、Bcl-2的表达水平。结果胃黏膜肠腺化生标本中,小肠型化生明显多于大肠型化生标本,不完全型小肠化生标本多于完全型小肠化生标本。在不完全型小肠化生标本中Ki67、p53、HER-2的表达,随着肠化生程度加重,其表达水平增高(P均<0.05)),尤其是Ki67在重度不完全型小肠化生中阳性程度亦逐渐增加(P<0.01)。而Bcl-2几乎无表达。结论 Ki67、p53、HER-2的异常表达与胃癌发生发展密切相关,Ki67有望成为肠腺化生恶变的一个监测指标。
Objective To explore the expressions and clinical significance of Ki67,p53, HER-2 and Bcl-2 in intestinal glands metaplasia of the gastric mucous membrane. Methods A total of 377 tissue specimens from atrophic gastritis combined with intestinal glands metaplasia in paraffin blocks in our hospital between January 2001and March 2009 were selected. The expressions of Ki67, p53, HER-2 and Bel-2 were detected by immunohistochemistry staining S-P method. Results Among the tissue speimens,the specimens of intestinal metaplasia type small intestine were more than the type large intestine, and the specimens of small intestinal metaplasia type non-complete were significantly more than the type complete. With the exacerbation of intestinal metaplasia degree, the expression levels of Ki67, p53, HER-2 gradually increased ( all P 〈 0. 05 ), and the positive expression degree of Ki67 in severe non-complete type small intestinal metaplasia also gradually increased (P 〈 0.01 ). The expression of Be1-2 in specimens of intestinal metaplasia of various type was hardly seen. Conclusions Abnormal expressions of Ki67,p53 and HER-2 are closely correlated with the development and progression of gastric carcinoma, and Ki67 might be served as one of the indicators monitoring canceration for intestinal metaplasia.
