摘要
目的:建立Ret-He、Hypo-He%、RBC-He、D-He、FRC%、FRC#、IRF、CHr及Hypo%的参考范围,并探索各参数在诊断贫血和监测骨髓移植中的临床意义。方法:采用XE-5000全自动血细胞分析仪检测401例表观健康人的Ret-He、Hypo-He%、RBC-He、D-He、FRC%、FRC#、IRF,用ADVIA 2120全自动血细胞分析仪检测CHr、Hypo%,建立上述各参数的参考范围;收集52例小细胞低色素性贫血和53例大细胞性贫血患者标本,评估上述各参数在两种类型贫血中的变化;动态观察9例骨髓移植患者的Ret%、Ret-He、CHr、IRF、RBC、HGB、WBC及PLT的变化,以寻找监测骨髓移植重建的早期指标;将意义相似但检测系统不同的参数进行相关性分析。结果:参考范围建立:Ret-He、IRF%、CHr为正态分布,参考范围分别为:35.5±2.4、9.63±6.7、31.7±1.7;RBC-He、Hypo-He%、FRC%、FRC#、D-He、Hypo%为偏态分布,参考范围分别为:29.6~33.7、0.1~0.6、0~0.35、0~0.0168、2.0~5.1、0.1~1.8。在小细胞低色素性贫血患者中,Ret-He、RBC-He和CHr降低,而Hypo-He%、FRC%、IRF、Hypo%升高;在大细胞性贫血患者中,Ret-He、Hypo-He%、RBC-He、D-He、FRC%、FRC#、IRF、CHr和Hypo%均升高。Ret-He与CHr、Hypo-He%与Hypo%的相关性均良好(r值分别为0.950、0.969,P<0.001)。IRF在移植术后10(7~13)d开始明显升高,较常规指标可提前5 d预测骨髓移植成功情况,而RBC、HGB、Ret-He、CHr在移植后25 d内没有明显变化。结论:Ret-He、RBC-He和CHr可用于小细胞低色素性贫血和大细胞性贫血的鉴别;IRF是监测移植重建的敏感指标。
Objective:To establish the reference ranges of Ret-He,Hypo-He%,RBC-He,D-He,FRC%,FRC #,IRF,CHr and Hypo%,and to study their clinical values in differentiating anemia and monitoring bone marrow transplantation.Methods: 401 samples from apparently healthy individuals were tested to establish the reference ranges of Ret-He,Hypo-He%,RBC-He,D-He,FRC%,FRC#,IRF by XE-5000 analyzer and the reference ranges of CHr and Hypo% by Bayer ADVIA 2120 analyzer.Further analyses on all of indices were performed in 105 samples from 52 patients with hypochromic microcytic anemia and 53 patients with macrocytic anemia.The clinical values of Ret-He,CHr,IRF,RBC,HGB,WBC,PLT and Ret% in monitoring hematopoietic recovery were also evaluated in 9 adult patients undergoing bone marrow transplantation.Results: The data of Ret-He,IRF% and CHr were normal distributed,and the reference ranges were 35.5±2.4,9.63±6.7,31.7±1.7,respectively.The data of RBC-He,Hypo-He%,FRC%,FRC#,D-He and Hypo% were non-normal distributed,and the reference ranges were 29.6~33.7,0.1~0.6,0~0.35,0~0.0168,2.0~5.1,0.1~1.8,respectively.Ret-He,RBC-He and CHr decreased in hypochromic microcytic anemia patients,while Hypo-He%,FRC%,IRF and Hypo% increased.In patients with macrocytic anemia,Ret-He,Hypo-He%,RBCHe,D-He,FRC%,FRC#,IRF,CHr and Hypo% all increased.Good linear correlation was found between Ret-He and CHr(r=0.950,P〈0.001) and between Hypo-He% and Hypo%(r=0.969,P〈0.001).It was observed that IRF first increased in 10d after transplantation and 5d in advance compared with routine parameters,while RBC,HGB,Ret-He and CHr had no change in 25d after transplantation.Conclusion: Ret-He,RBC-He and CHr could be used to differentiate hypochromic microcytic anemia from macrocytic anemia,and IRF could be used to monitor hematopoietic recovery after bone marrow transplantation.
