摘要
目的探讨原发性胆囊癌、胆囊腺瘤样息肉和慢性胆囊炎组织中组织蛋白酶B（CatB）及组织蛋白酶D（CarD）的表达和相关性及其与临床病理特征之间的关系。方法采用免疫组织化学染色方法检测32例胆囊癌组织和20例胆囊良性病变（其中胆囊腺瘤样息肉10例，慢性胆囊炎10例）组织中CatB及CatD的表达。结果CatB、CatD在胆囊癌组织的阳性表达率分别为93．75％（30／32）、90．63％（29／32），均明显高于胆囊腺瘤样息肉组织（分别为60．00％、60．00％，P〈0．05）和慢性胆囊炎组织（分别为60．00％、50．00％，P〈0．05）。CatB、CatD的表达在患者年龄、性别、标本Nevin分期、病理学分级、病理学类型间的差异均无统计学意义（P〉0．05）。CatB与CatD的表达呈正相关性（r=0．434，P〈0．05）。结论CatB和CatD在胆囊癌组织中表达增强，可作为胆囊癌组织学标志物。胆囊组织中CatB和CatD的高表达与临床病理特征无明显相关。CarD与CatB的表达呈正相关。
Objective To investigate the expression of cathepsin B （CatB） and cathepsin D （CatD） in primary gallbladder carcinoma, adenomatoid polyp without malignant transformation, and chronic cholecystitis tissues and to assess their mutual correlation ships as well as the relationships between their expressions and clinical pathological features. Methods Detected the expressions of CatB and CatD in gallbladder tissues including 32 specimens of primary gallbladder carcinoma, 10 specimens of adenomatoid polyp without malignant transformation and 10 specimens of chronic cholecystitis by immunohistochemical method . The expressions were assessed by semi-quantitative analysis. Results The positive rate of CatB, CatD and CCL20 in primary gallbladder carcinoma was 93.75% （30/32） and 90. 63% （29/32） respectively, being significantly higher than those in adenomatoid polyp without malignant transformation （60% and 60% , P 〈0. 05） and chronic cholecystitis groups （60% and 50% P 〈0. 05） . The expression levels of CatB and CatD were not significantly related to the clinical features including age and gender, or pathological features including grades of cell differentiation, pathological types and Nevin stages （ P 〉 0. 05 ）. CatB and CatD had significantly positive correlations （ r = 0. 434, P 〈 0.05 ）. Conclusion （ 1 ） CatB and CatD CCL20 might serve as the histological marker of primary gallbladder carcinoma. （ 2 ） The expression levels of CatB and CatD in primary gallbladder carcinoma are not related to the clinical or pathological fea- tures. （ 3 ） There is positive correlation between the expression levels of CatB and CatD in primary gallbladder carcinoma.
