摘要
目的探讨益肾化瘀透邪方治疗狼疮性肾炎(LN)临床疗效及对并发感染的影响。方法将60例LN患者随机分为2组,治疗组30例用益肾化瘀透邪方联合强的松、环磷酰胺治疗;对照组30例用强的松、环磷酰胺治疗,2组均以4周为1个疗程,6个疗程后观察疗效。观测患者系统性红斑狼疮疾病活动指数(SLEDAI)评分、血白细胞计数、红细胞沉降率(ESR)、C反应蛋白(CRP)、补体C3、抗ds-DNA抗体、血白蛋白、24 h尿蛋白定量、尿红细胞计数、肝功能、肾功能等指标,同时观察感染发生情况及其他不良反应。结果 2组治疗后SLEDAI评分、血白细胞计数、ESR、CRP、补体C3、抗ds-DNA抗体、血白蛋白、24 h尿蛋白定量、尿红细胞计数等各项观察指标均有明显改善,治疗前后差异有统计学意义(P<0.05);治疗组和对照组总有效率分别为90.0%(27/30)和73.3%(22/30),2组比较差异有统计意义(P<0.05)。治疗组感染率26.7%(8/30),对照组感染率46.7%(14/30),2组比较差异有统计意义(P<0.05)。结论益肾化瘀透邪法对LN的疗效显著,可降低LN患者感染率。
Objective To observe the efficacy of Yishen Huayu Touxie Decoction on lupus nephritis（LN） and its effect on complicated infection.Methods Sixty LN patients were randomly divided into two groups,30 patients in the treatment group were treated with Yishen Huayu Touxie Decoction combined with prednisone and cyclophosphamide,and 30 patients in the control group were treated with prednisone and cyclophosphamide.Both of the two groups were treated for 4 weeks as a course of treatment.After 6 courses of treatment,the university of Toron to SLE disease activity index（SLEDAI）,white blood cell count,the erythrocyte sedimentation rate（ESR）,C-reactive protein（CRP）,complement C3,double stain DNA antibody,blood albumin,24-hour urine protein,red blood cell count of urine,liver function,renal function,and the infection rate as well as side effect of medicine were observed.Results SLEDAI,white blood cell count,the levels of ESR,CRP,C3,double stain DNA antibody,blood albumin,24-hour urine protein,red blood cell count of urine in the two groups were improved obviously after treatment,the differences between pretreatment and post treatment were significant（P〈0.05）.Total effective rate of treatment group and control group was 90.0%（27/30） and 73.3%（22/30）,the difference between the two groups was significant（P〈0.05）.The infection rate of treatment group and control group was 26.7%（8/30） and 46.3%（14/30）,the difference between the two groups was significant（P〈0.05）.Conclusion Yishen Huayu Touxie Decoction has significant efficacy on LN patients,and can reduce the infection rate.
