抽出钢丝悬吊法治疗指伸肌腱中央束止点撕脱伤 被引量:17

Treatment of extensor central slip avuision injury with traetien wire procedure

摘要 目的探讨抽出钢丝悬吊法治疗指伸肌腱中央束止点撕脱伤的临床疗效,为其治疗提供新的方法。方法采用抽出钢丝悬吊法治疗指伸肌腱中央束止点撕脱伤12例,患者平均年龄30岁。受伤至手术时间平均14d,均为闭合性损伤,其中3例合并中节指骨基底背侧撕脱骨折。术后石膏外固定6周后拔除克氏针并抽出钢丝,开始进行功能锻炼。结果术后平均随访时间为9个月(3~15个月),3例骨折片均获得完全复位,骨折临床愈合时间平均为7周(6—8周)。术后测定患指屈伸功能:远指间关节主动伸直平均-4°(-10°~0°),主动屈曲平均45°(30°~70°);近指间关节主动伸直平均-9°(-20°-0°),主动屈曲平均81°(72°~90°)。根据中华医学会手外科学会上肢部分功能评定试用标准:优10例,良1例,可1例;优良率达91.7%。结论抽出钢丝悬吊法是治疗指伸肌腱中央束止点撕脱伤的有效方法。 Objective To investigate the clinical outcomes of treatment of extensor central slip avulsion injury with traction wire procedures. Methods Twelve cases of extensor central slip avulsion injuries were treated with traction wire procedures. The average age of the patients was 30 years (range, 16 to 45 years) and the average duration from injury to treatment was 14 days (range,7 to 30 days). All the cases sustained closed injuries, among them there were 3 cases with avulsion fractures. The wires were, removed 6 weeks postoperatively when rehabilitation started. Results The postoperative follow-up period was 9 months on average (range, 3 to 15 months). Union of all the avulsion fractures was obtained within 7 weeks (range,6 to 8 weeks). The functions of the involved fingers were evaluated as following: the mean active extension and flexion of DIP joints were -4° (range, - 10° to 0°) and 45°(range, 30°to 70°), respectively; the mean active extension and flexion of PIP joints were -9°(range, -20° to 0°) and 81°(range, 72° to 90°), respectively. According to the Upper Extremity Funetional Evaluation System of Chinese Hand Surgery Association, the treatment outcomes were excellent in 10 eases, good in one ease and fair in one ease. The excellent and good rate reached 91.7%. Conclusion Traction wire procedure is a reliable treatment method for extensor central slip avulsion injury.
