摘要
目的 :探讨逆行灌注心脏不停跳法心肌保护作用的效果。方法 :用离体兔心工作模型 ,经冠状静脉窦逆行灌注稀释血液或血钾停跳液 12 0 m in。 40只长耳大白兔随机分成 4组 ,每组 10只 ,A组 (对照组 ) ,B组 (心脏不停跳组 ) ,C组 (冷血钾间断灌注心停跳组 ,血温 15℃ ) ,D组 (温血连续灌注心停跳组 ,血温 32℃ )。观察心功能、左右心室心肌生化、氧自由基、Na+、Ca2 +离子、含水量和超微结构的变化。结果 :B组左室收缩压恢复率 91.6 9% ,三磷酸腺苷 (ATP)含量、超氧化物歧化酶 (SOD)活性高于 C、D组 (P<0 .0 5 ) ;丙二醛 (MDA)、 Na+含量低于 C、 D组 (P<0 .0 5 ) ;心肌超微结构无明显改变 ;左右室心肌各项指标变化差异无显著性 (P>0 .0 5 )。结论 :逆行灌注心脏不停跳法对左右心室均有良好的保护作用 ,是一种安全、较接近生理状态的心肌保护的新方法。
Objective:To evaluate the myocardial protective effects on retrograde coronary sinus perfusion in beating heart Methods:Isolated rabbit working heart models were used in this study Fourty 3~4 months rabbits were rondomly divided into 4 groups which were Group A (control group,n=10),Group B (beating heart,n=10),Group C (cold blood cardioplegia arrest at 15℃,n=10),Group D (continuous warm blood cardioplegia arrest,n=10 ) Cardiac functional recovery,changes of myocardial biochemistray,free radical generation,Na +,Ca 2+ and ultrastructure in left and right ventrical were assessed after 120 minutes continuous perfusion with retrograde coronary sinus Results:Cardical function recovery,ATP,superoxide dismutase (SOD) and ultrastructure were superior to those in Group B than Group C and D The levels of Malondialdehyde(MDA),Na + were lower singnificantly in Group B than Group C and D (P<0 05) There were no difference in left and right ventrical on those changes in Group B (P>0 05) Conclusion:This methose is of the same efficiency for myocardial protection in left and right ventricals,and is myocardical protective way close to physiologic state
