摘要
连续性照护是随着时间和地点的变化,调动医院、社区和家庭等多方面力量,共同为患者提供连续、全方位的服务,保证患者康复治疗效果。连续性照护作为一种基本的管理模式,在脑卒中患者康复护理中显现出很多优势。本文从连续性照护的概念、核心领域及国内外脑卒中患者连续性照护干预策略等方面阐述,并对该模式应用现状进行评价和远景展望,以期为我国脑卒中疾病管理提供参考。
With the change of time and places,the continuity care is to mobilize hospitals,communities,families,and other resources to provide continuous,omnidirectional services for patients to ensure the effects of rehabilitation.As a basic disease management model,continuity care has many advantages in stoke rehabilitation nursing care.This paper described the conception,the core areas and the intervention strategies about continuity care for stroke patients at home and abroad,and evaluated prospected the mode-application,in order to provide references for stroke management.
