经皮钢板固定治疗胫骨远端骨折

Treatment of Distal Tibial Fractures Using Percutaneous Plate Osteosynthesis

摘要 目的:介绍微创经皮插入接骨板内固定治疗胫骨远端骨折,并观察其效果。方法:采用闭合复位经皮插入接骨板内固定治疗胫骨远端骨折36例。AO分型:A型23例(A2型9例,A3型14例),B型9例,C型4例。术后不辅以其他外固定,第2天即开始不负重关节功能锻炼。结果:所有患者随访6~22个月,平均11个月。随访36例,所有病例伤口一期愈合。骨折无延迟愈合,无畸形愈合,平均临床愈合时间17周。根据Mazur的踝关节功能评分,优良率94.4%。结论:微创经皮插入接骨板内固定术是治疗胫骨远端骨折的有效方法。 Objective:To introduce minimally invasive percutaneous plate osteosynthesis(MIPPO) for the treatment of distal tibial fractures and investigate the results.Methods:36 cases of distal tibial fractures were treated with MIPPO.According to the AO classification,there were 23 cases of type A(9 type A2,14 type A3),9 type B and 4 type C.Without any external fixation,active and passive motion of ankle joint was allowed two days after operation without weight bearing.Results:All the patients were followed up for 6~22 months,11 months on average.Followed up 36 cases.The mean time of bone union was 17 weeks.No infection.Delayed union,nonunion or malunion was found in this Series.According to Mazur ankle joint function scoring,the excellent and good rate was 94.4%.Conclusion:MIPPO is a safe and effective technique in the treatment of distal tibial fracture.
