阻塞性睡眠呼吸暂停低通气综合征上气道双源CT和测压检查诊断价值的对照研究 被引量:5

摘要 目的对照分析上气道双源CT和测压两种检查评价阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的应用特点及各自优势。方法通过上气道间隙双源CT狭窄判定结果与测压结果进行比对,分析两种检查方法对OSAHS的应用优势和区别。结果双源CT检查判定OSAHS患者上气道狭窄平面的阳性率为83.33%(25/30),能比较准确反映上气道的解剖性狭窄状况。睡眠监测阻塞定位仪(AG200)判定OSAHS患者上气道间隙阻塞的阳性率为90.00%(27/30),能反映上气道的功能阻塞状况。结论双源CT检查和AG200对OSAHS患者上气道阻塞均具有诊断意义,CT检查可以反映上气道多平面联合狭窄和上气道解剖性狭窄的原因,但对部分患者睡眠期潜在的气道塌陷引起的狭窄存在局限性;AG200可以发现睡眠期气道功能阻塞状况,但仅能单纯显示单一平面的功能阻塞情况。 Objective To evaluate characteristics and advantages of the dual source CT and airway manometry in OSAHS. Methods Through the upper airway narrow the gap to determine the results of dual source CT results were compared with the pressure measurement, advantages and differences of two methods were analyzed. Results Positive rate of dual source CT scan to determine the upper airway stenosis in patients with OSAHS was 25/30 plane, which could accurately reflect the anatomy of the upper airway stenosis condition. Positive rate of AG200 to determine obstruction in OSAHS patients was 27/30, which reflected the function of upper airway obstruction condition. Conclusion Both dual source CT and AG200 of the upper airway obstruction in patients with OSAHS have diagnostic value, CT examination can reflect the multiplanar narrow airway and reasons for upper airway anatomic stenosis, but it has limitation in some patients with potential collapse of the airway ; AG200 can be used in functional obstruction of airway during sleep state, but it only display a single plane obstruction.
