摘要
目的 :对照观察留行子耳穴按压对术后切口疼痛的疗效 ,并探讨其镇痛机理。 方法 :手术后切口疼痛患者 6 0例 ,随机分成两组 ,治疗组 30例采用留行子按压耳穴 ,对照组 30例应用吗啡类镇痛药 ,比较两组的镇痛疗效及术后并发症。用荧光分光光度法测定两组治疗前、治疗后 6h、2 4h的血浆 5 HT浓度变化。 结果 :(1 )治疗组镇痛疗效略高 ,但两组差异无显著性 (P >0 .0 5 ) ;(2 )治疗组术后并发症 (肺部感染 ,尿潴留 )发生率显著低于对照组 (P <0 .0 1 ) ;(3)治疗组治疗后 6h、治疗后 2 4h血浆 5 HT显著高于对照组 (P <0 .0 1 ) ;(4)治疗组治疗后 6h血浆 5 HT显著高于治疗前 ,治疗后 2 4h血浆 5 HT显著高于治疗后 6h(P <0 .0 1 )。 结论 :留行子耳穴按压疗法对术后切口疼痛有确切的镇痛作用 ,可与吗啡类相比拟 ,并可减少术后并发症 ;治疗后血浆 5 HT水平显著升高 ,可能是耳穴按压止痛的机理之一。
Objective:To assess the effects of PMAP with Liu xingzi on postoperative wound pain and explore its analgestic mechanism.Methods:Sixty patients with postoperative wound pain were randomized to divide into two groups.Thirty patients were treated by PMAP to relieve wound pain(as control group).The efficacy and postoperative complications in both groups were observed and compared.Plasma 5 HT levels in both groups were determined respectively before treatment and at 6th and 12th hour after treatment by fluorescence technique.Results:1.Though the effect of the treatment group was slightly better than that of the control , there was no significant difference between two groups ( P>0.05 ).2.The incident of the postopertive complications(eg.pulmonary infection,retention of urine)in the treatment group was significantly lower than that in the control( P<0.01 ).3.Plasma 5 HT levels at 6th and 24th hour after treatment in the treatment group increased significantly compared to the control( P<0.01 ).4.Plasma 5 HT level at 6th hour after tretment was significantly higher than that before treatment( P<0.01 ).Plasma 5 HT level at 24th hour after treatment was significantly higher than that at 6th hour after treatment( P<0.01 ).Conclusion:1.PMAP has definite effect comparable to morphine in relieving postopertive wound pain and can reduce the incident of postopertive complications.2.The increase of the plasma 5 HT level may be an important analgestic mechanism of PMAP.
