摘要
目的探讨HPV16/18感染和p53基因多态性与唐山地区食管癌的关系。方法收集唐山地区食管癌患者45例,食管良性肿瘤20例,健康对照组20例,采用原位杂交法检测HPV16/18,用PCR-RFLP方法检测p53基因多态性,对照分析其与食管癌发生率的关系。结果食管癌组患者、食管良性肿瘤与健康对照组HPV16/18检出率分别为88.9%、5.0%、2.5%,食管癌组患者HPV16/18检出率明显高于其他组(P<0.05)。p53基因Arg/Arg基因型频率在食管癌组患者、食管良性肿瘤组患者与健康对照组患者中分别为42.2%、35.0%、20.0%,差异有统计学意义(P<0.05)。在患者癌组织中三种基因型Pro/Pro、Arg/Arg和Pro/Arg构成比在HPV16/18感染组和未感染组差异有统计学意义(P<0.05)。结论唐山地区食管癌高发区可能与高危型HPV16/18的感染有关,个体携带p53 Arg/Arg基因型更容易发生HPV16/18相关的食管癌。
Objective To study the relationship between HPV16/18 infection and p53 polymorphisms with esophageal cancer in Tangshan area.Methods A case-control study was performed in 45 esophageal cancer patients,20 esophageal benign tumor patients and 20 healthy controls in Tangshan area.HPV16/18 were detected in situ hybridization,polymorphism of p53 were detected by PCR.The relationship between HPV16/18 infection and polymorphism of p53 with the esophageal cancer in incidence rate was analyzed.Results The positive rate of HPV16/18 in esophageal cancer group(88.9%) in Tangshan was higher than another group(P 0.05).The frequency of Arg/Arg gene type on codon of p53 in esophageal caccer group(42.2%) was higher than that in esophageal benign tumor group(35.0%) and healthy controls group(20.0%),the difference was statistically significant(P 0.05).The incidence rate of three gene types(Pro/pro,Arg/Arg,Pro/Arg) in HPV16/18 positive groups was higher than negative group in esophageal cancer group,the difference was statistically significant(P 0.05).Conclusion High risk HPV16/18 plays an important role in development of esophageal cancer in Tangshan area.P53 Arg/Arg gene type is one of the high risk genetic factors for HPV-associated esophageal cancer in Tangshan area.
