妊娠期高血压疾病血胱抑素C变化的研究 被引量:7

Study on the Change of Cystatin C Level in Hypertensive Disorder Complicating Pregnancy

摘要 目的探讨血清胱抑素C水平变化与妊娠期高血压疾病的关系,评估胱抑素C对妊娠期高血压疾病诊断价值。方法随机抽取2011年7月-2012年1月湖北省妇幼保健院住院分娩孕产妇为研究对象。收集妊娠期高血压疾病253例,同时选择无妊娠合并症与并发症健康孕妇250例作为对照组,检测孕妇血清胱抑素C(CysC)、&微球蛋白(β2-MG)、尿素氮(BUN)、尿酸(UA)及C-反应蛋白(cRP)水平进行比较。结果妊娠期高血压疾病组各项指标水平均高于对照组,且差异有统计学意义(P〈0.05)。妊娠期高血压疾病组血清CysC与β2-MG、UA水平呈正相关,与β2-MG相关性最强(r=0.586,P〈0.05)。CysC、β2-MG、UA与BUN的ROC曲线下面积(AUC)分别为0.862(95%CI为0.734-0.923)、0.763(95%CI为0.652-0.822)、0.742(95%CI为0.527-0.718)和0.704(95%CI为0.421-0.615),差异有统计学意义(P〈0.01)。结论胱抑素c在评价妊娠期高血压疾病早期肾损害中起重要作用。 Objective To explore the relationship between serum cystatin C level and hypertensive disorder complicating pregnancy (HI)CP), and to evaluate the diagnostic value of cystatin C in HDCP. Methods Two hundred and fifty- three ly- ing- in women with HDCP hospitalized in Hubei Provincial Maternal and Child Health Hospital from July 2011 to January 2012 were randomly selected as research objects. Meanwhile, 250 healthy lying- in women were selected as the controls. The levels of Cys - C, β2- MG, BUN, UA, and CRP were detected and compared between the two groups. Results The levels of Cys - C, β2- MG, BUN, UA, and CRP in HDCP group were all higher than those of control group, and the difference was statistical- ly significant (P 〈 0.05). The level of serum CysC in HI)CP group was positively correlated with β2- MG and UA levels, more- over, it was the mostly associated with β2- MG level ( r = 0. 586, P 〈 0.05). The areas under ROC curves of Cys - C, β2- MG, BUN, UA, and CRP were 0. 862 (95 % CI = 0. 734- 0. 923), 0. 763 (95 % CI = 0. 652- 0. 822), 0. 742 ( 95 % CI = 0. 527 0. 718), and 0. 704 (95 % CI = 0. 421 - 0. 615 ), respectively, and the difference was statistically significant ( P 〈 0.01 ). Conclusion Serum CysC plays an important role in the evaluation of early renal damage of HI)CP.
