摘要
目的 :优选对伤口损伤小、疗效好、费时少的伤口护理方法。方法 :将 6 3例病人计 77处伤口 (褥疮、糖尿病足溃疡、烫伤、药物性溃疡 )分为A、B、C 3组 ,采用“生理盐水涡流式冲洗”或“双氧水涡流式冲洗 +生理盐水涡流式冲洗”清洁伤口后 ,分别给予自制敷料封闭胰岛素湿敷 +口服常用量维生素C和自制敷料 /进口敷料封闭胰岛素湿敷 +口服大剂量维生素C进行对比治疗 ,观察了疗效并比较了自制敷料和进口敷料的效果及费用。结果 :进口敷料封闭胰岛素湿敷加服大剂量维生素C组的Ⅱ期伤口愈合率高且平均愈合时间短 ,换药次数及时间明显减少而平均费用无明显增加。结论 :采用“涡流式水流冲洗伤口”后 ,用进口敷料封闭胰岛素湿敷 ,加服大剂量维生素C ,可缩短各类伤口的愈合时间 ,节省护理时间而不增加病人费用。
Objective:To choose a method of wound care which had less side effects,better effects and shorter healing time.Method:In this study,63 patients with 77 wounds were registered and accepted different management.According to the assessment results of wounds and their nutrient condition,patients were divided into three groups:A including 20 patients with 24 wounds,B including 21 patients with 25 wounds and C incluidng 22 patients with 28 wounds.These wounds were irrigated with whirling fluid of 0.9% NaCl and 3% H 2O 2 respectively,and then were sprayed insulin,at last were covered with occlusive dressings of which one was imported and the other was made by ourselves.Meanwhile,of two kinds of dressing and of two doses of vitamine C were compared.Result:In group C,the rate of curing wounds was the highest and the average healing time was the shortest.Moreover the average cost did not increase.Conclusion:The method of irrigation wounds with whirling fluid,and covering them by insulin and the imported occlusive dressing,giving great doses Vitmine C was the most effective.
