胸部肿瘤放射治疗两种体位固定技术摆位误差比较研究 被引量:29

Setup errors of two position fixing skills in radiotherapy for thoracic neoplasm

摘要 目的:测量胸部肿瘤患者接受放射治疗时两种不同体位固定技术的摆位误差,对两种体位固定技术的准确性和重复性进行比较。方法:收集70例胸部肿瘤(食管癌、肺癌、纵隔转移淋巴结等)患者,采用区组随机化分组随机分为试验组与对照组,试验组采用翼形板+真空垫固定体位,对照组采用真空垫固定体位。在治疗过程中每个患者通过CBCT测量5次摆位误差。结果:与单纯使用真空垫固定体位组相比,使用翼形板+真空垫可明显减小Z轴(头脚方向)的摆位误差(2.3±1.6/3.0±3.4,P<0.05),但未减小X轴(左右方向,1.8±1.2/1.8±1.1,P>0.05)、Y轴(上下方向,2.3±1.6/2.4±1.7,P>0.05)的摆位误差。结论:采用翼形板+真空垫体位固定技术可减小胸部肿瘤放疗的摆位误差,提高了定摆位的精度。 OBJECTIVE:To explore the setup errors of two different position fixing skills in radiotherapy for thoracic neoplasm patients, and compare the accuracy and repetitiveness of the two different position fixing skills. METHODS: Sev- enty thoracic neoplasm(esophageal cancer,lung cancer,mediastinal shift lymph node etc) patients were colleted. Patients using block were divided into the experimental group and control group vandomly. The experimental group were treated u- sing wing shape plate+ vacuum mat fixed position, and the control group by vacuum mat fixed position. Both groups were measured the setup errors during therapy by CBCT (Cone-beam CT). RESULTS: Compared with the control group, the experimental group can obviously reduce set up errors in Z axis (2.3±1.6/3.0±3.4,P〈0.05) ,but not in X axis (1.8± 1.2/1. 8± 1.1, P〉0. 05) and Y axis ( 2.3 ± 1.6/2.4 ±1.7, P〉0. 05). CONCLUSION : Using wing shape plate+ vacuum mat fixed technology can reduce the setup errors in radiotherapy for thoracic neoplasm, and improve the precision of a fixed olace.
