摘要
目的探讨老年患者下呼吸道感染病原菌分布及耐药现状,为临床合理用药提供帮助。方法对2008年1月-2010年12月老年患者下呼吸道感染痰标本所分离的病原菌进行菌种分布和耐药性回顾性统计分析。结果共检出病原菌1147株,其中革兰阴性菌837株占72.97%,前4位分别为肺炎克雷伯菌288株占25.11%、铜绿假单胞菌217株占18.92%、鲍氏不动杆菌151株占13.16%和大肠埃希菌103株占8.98%;革兰阳性菌235株占20.49%,前3位分别为金黄色葡萄球菌128株占11.16%、溶血葡萄球菌61株占5.32%和肠球菌属30株占2.62%;真菌75株,占6.54%;万古霉素对革兰阳性菌高度敏感,亚胺培南对革兰阴性菌高度敏感。结论老年患者下呼吸道感染病原菌以革兰阴性杆菌为主,及时了解痰培养结果可以对临床抗菌治疗提供依据。
OBJECTIVE To explore the distribution and antibiotics resistance of the pathogens isolated from the sputum of elderly patients with lower respiratory tract infections so as to guide the rational application of antibiotics. METHODS A retrospective analysis of the distribution and antibiotics resistance of pathogens isolated from sputum of elderly patients with lower respiratory tract infections was made from Jan 2008 to Dec 2010. RESULTS Totally 1147 strains of pathogens were isolated from the sputum of elderly patients with lower respiratory tract infections, including 837 strains of gram-negative bacilli （72.97%）, 235 strains of gram-positive bacteria （20.49%） ,and 75 strains of fungi （6.54%）. The top four species of gram-negative bacteria were Kleb-siella pneumoniae （ 288 strains （25. 11%）, Pseudomonas aeruginosa （ 217 strains, 18. 92 %）, Acinetobacter bau- mannii （151 strains, 13.16%） and Escherichia coli （103 strains,8.98% ）. The top three species of gram-positive bacteria were Staphylococcus aureus （128 strains, 11. 16%）, S. haemolyticus （ 61 strains, 5. 32%）, and Enterococci （30 strains,2. 62%）. The gram-positive bacteria were highly susceptible to vancomycin, the gram-negative bacteria were highly susceptible to imipenem. CONCLUSION The gram-negative bacilli are the main pathogens causing lower respiratory tract infections in the elderly patients. To understand the sputum culture result in a timely manner can provide evidence for the clinical antibacterial treatment.
