摘要
目的比较动力髋螺钉、锁定钢板、外固定支架3种固定方法治疗老年股骨转子间骨折的疗效疗效。方法回顾性分析2006年8月至2011年8月期间我院120例老年股骨转子骨折患者病例资料,根据治疗方法分为动力髋螺钉组(49例)、锁定钢板组(38例)与外固定架组(33例),比较各组手术方法、手术时间、出血量、下地负重时间、骨折愈合时间、固定失败率及术后半年时患者关节Harris评分情况。结果术后随访时间7～24个月,平均(16.32±6.24)个月,动力髋螺钉组、锁定钢板组手手术时间、出血量、骨折愈合时间较外固定支架组长,Harris评分较外固定支架组高,比较差异存在统计学意义(P<0.05)。结论三种固定方法各有其自身特点,均是治疗老年股骨转子间骨折的有效方法,锁定钢板、动力螺钉治疗手术损伤、骨折愈合时间较外固定支架长,但可早期下地负重活动,外固定支架手术时间、术中出血量明显少,术中损伤少,骨折愈合时间短,避免二次手术取出内固定,尤其适用于高龄、体质弱的老年股骨转子间骨折。
Objective To compare dynamic hip screw,locking plate,fixing support three fixed method to treat old femur rotor between the effect of fracture curative effect.Methods Retrospective analysis in August 2006-August 2011 period in our hospital from 120 elderly cases of femoral fractures were treated with rotor case material,according to the treatment methods are classified into dynamic hip screw group（49 cases）,locking plate group（38 cases） and external fixator group（33 cases）,compare the surgical method,operation time,amount of bleeding,and load time,fracture healing time,fixed failure rate and postoperative half a year in patients with joint Harris score situation.Results Postoperative follow-up time 7～24 months,an average of（16.32＋/-6.24）months,dynamic hip screw locking plate in a group,the group of hand operation time,amount of bleeding,fracture healing time is the fixed support group leader,Harris score more external fixed support group is high,the comparative differences exist statistical significance（P 0.05）.Conclusion Three fixed method each have their own characteristics,are the treatment of senile femoral fracture between rotor effective method,locking plate,power screws for the treatment of surgical injury,fracture healing time is the fixed support long,but the earth early weight activities,external fixed support operation time,amount of bleeding during the operation,the less obvious,intraoperatie injury less,fracture healing time short,avoid reoperation remove internal fixation,especially suitable for older age,physical weak elderly femoral fracture between rotor.
