摘要
目的探讨老年人死亡引发医疗纠纷的原因及其病理死因。方法回顾性分析我院2006—2011年受乐山市医疗事故鉴定专家委员会委托进行医疗纠纷尸检案例中的16例老年患者的临床资料。结果 16例死亡发生在三级医院4例,二级及以下医院12例。本组9例明确临床死因,与病理死因相符5例,不相符4例;余7例无明确临床死因诊断,后经尸检证实病理死因。出具病理死因尸检结果后,经市级医疗事故鉴定委员会鉴定,4例为一级甲等医疗事故,4例存在明显诊疗失误,医方主动承担责任并与患方协商解决;8例因疾病发展造成死亡,医方无诊疗过失,患方主动撤诉。结论尸检能查明死因,有助于提高临床诊治水平,解决医疗纠纷。对医疗纠纷的死亡病例进行尸检,既可维护医疗机构和医务人员的权益,又可保护患者的利益。
Objective To explore the pathological death cause and the cause of medical dispute of elderly patients. Methods A retrospective analysis of 16 medical malpractice autopsy of elderly patients in our hospital from 2006 to 2011 commissioned by the Mal- practice Appraisal Expert Committee of Leshan City were performed. Results 16 deaths were reported 4 in tertiary hospitals and 12 in second class or below second class hospitals. There were 9 cases with definite clinical cause of death, in which clinical death and patho- logical death were matched in 5 cases and not matched in 4 cases; the remaining 7 patients without clear diagnosis were confirmed of pathological cause of death by autopsy. Autopsy results issued by the municipal malpractice appraisal committee in 4 cases of first class grade A malpractice were obviously due to medical errors, and the hospitals were taken responsible and solved the disputes through ne- gotiation ; In 8 cases due to disease development and not doctors＇negligence, the families withdrew law suits. Conclusion Autopsy may identify the cause of death and contribute to the solution of medical disputes. Concerning medical dispute of death cases, autopsy can not only protect the rights and interests of medical institutions and medical workers, but also protect the interests of the patients.
