摘要
目的 分析类风湿关节炎 (RA)患者滑膜液及外周血中Th1/Th2型细胞因子网络的偏移及其在疾病过程中的意义。方法 用酶联免疫斑点法 (enzymelinkedimmunospotassay ,ELISPOT) ,检测RA患者滑膜液单个核细胞 (SFMC)和外周血单个核细胞 (PBMC)中的Th1和Th2细胞 ,并分析Th1/Th2细胞的比值与血沉 (ESR)和C 反应蛋白 (CRP)的相关性。结果 与PBMC相比较 ,RA患者SFMC中CD3 +T细胞的百分率 ,Th1细胞及Th1/Th2细胞的比值显著升高P <0 0 1) ,且此比值与患者的ESR和CRP均呈正相关(分别为r =0 84,P <0 0 1和r =0 74,P <0 0 2 )。结论 在RA患者关节中Th1细胞明显增多并占优势 ,Th1/Th2细胞的比值与疾病的活动程度明显相关 。
Aim To analyize the shift of Th1/Th2 type cytokine profile in synovial fluid and peripheral blood from patients with rheumatoid arthritis (RA) and its clinical significance. Methods Th1 and Th2 cells in synovial fluid mononuclear cells (SFMC) and paired peripheral blood mononuclear cells (PBMC) from 12 RA patients were detected by enzyme linked immunospot assay (ELISPOT). Correlation of Th1/Th2 cell ratio with erythrocyte sedimentation rate (ESR) and Creactive protein(CRP) were analyized. Results The mean percentages of CD3+ T cells,Th1 cells and Th1/Th2 cell ratio were significantly increased in the SFMC as compared with PBMC(P<001). The Th1/Th2 cell ratio of SFMC in RA patients was correlated significantly with ESR and CRP(r=084, P< 001 and r=074,P<002, respectively). Conclusion The Th1 cells in joints of RA patients increased and predominated. Th1/Th2 ratio correlated markedly with the disease activity.Th1/Th2 cell balance has important implications for the pathogensis of RA.
