摘要
目的分析双吻合器技术在中低位直肠癌保肛术中的临床应用体会。方法选择内蒙古包钢医院2009年1月至2012年6月89例中低位直肠癌患者为研究对象,根据随机分组原则分为对照组(传统手术组)45例及观察组(双吻合器技术组)44例,两组患者均要求保留肛门,比较手术效果。结果观察组病例保肛成功率为100%,明显高于对照组的86.7%(P<0.05);手术时间平均为(86.4±18.3)min、住院时间平均为(21.5±6.7)d、吻合口瘘发生率为2.3%,均明显低于对照组的(98.1±15.7)min、(26.2±7.2)d、11.1%(P<0.05);吻合口狭窄发生率为2.3%、复发率为4.5%,与对照组的11.1%、0比较差异无显著性(P>0.05)。结论在中低位直肠癌保肛手术中应用双吻合器技术能够明显提高保肛成功率,降低手术时间及住院时间、吻合口瘘发生率,操作简单方便。
Objective To analyze the clinical application of double stapler technique in anus preservation surgery for the treatment of low rectal cancer.Methods From January 2009 to June 2012,89 patients with low rectal cancer were selected as objects in our hospital.They were randomly divided into observation group and control group.Control group（conventional surgery group,n=45） and observation group（double stapler group,n=44）.The patients required retaining anus and we made a comparison on clinical effect.Results In observation group,anus preservation success rate was 100%,it was significantly higher than the control group,P0.05;The average operation time was（86.4 ± 18.3） minutes,the average hospitalization was（21.5 ± 6.7） days,anastomotic fistula rate was 2.3%,they were significantly lower than the control group（P0.05）.Anastomotic stricture rate was 2.3%,the relapse rate was 4.5%,when compared with the control group,there were no differences（P0.05）.Conclusion The double stapler technique in anus preservation surgery for the treatment of low rectal cancer can significantly improve the success rate,reduce the surgery and hospitalization time,anastomotic fistula rate,the operation can be simple and convenient.
