机械通气危重症患儿血清白蛋白水平变化对预后的影响 被引量:10

摘要 目的探讨机械通气危重症患儿血清白蛋白水平对预后的影响。方法 2011年1月1日—2011年7月1日收集机械通气危重症患儿85例,观察其不同白蛋白水平与小儿危重症评分(PICS)评分的相关性以及对患儿预后的影响。结果机械通气危重症患儿低蛋白血症的发生率为63.5%,不同PICS评分组患儿间的血清白蛋白水平的差异均有统计学意义(P均<0.05)。不同白蛋白水平患儿机械通气时间、住院和住PICU时间、合并呼吸机相关性肺炎及病死率等的差异也均有统计学意义(P均<0.05)。结论血清白蛋白水平可作为评估机械通气危重症患儿病情严重程度和病死率的一个重要指标。 Objectives To explore the relationship of serum albumin levels and the prognosis of critically ill children undergoing mechanical ventilation. Methods Eighty-five critically ill children undergoing mechanical ventilation were enrolled from January 1, 2011 to July 1, 2011. The relationships of different serum albumin levels with PICS score and prog- nosis were studied. Results The incidence of hypoalbuminemia in critically ill children undergoing mechanical ventilation was 63.5%. The study showed statistical significances between PICS scores and serum albumin level (all P〈0.05). The study also showed statistical significances between albumin level and mechanical ventilation dependency, hospital stay and PICU stay, complicated ventilator-associated pneumonia and mortality respectively (all P〈0.05). Conclusions Serum albumin levels can be used as one of the important indexes to evaluate severity of illness and mortality.
