摘要
目的观察改良体位垫在椎体形成术中的应用效果。方法将在本院俯卧位下行初次椎体形成术手术的60例患者随机分为实验组与对照组,实验组术中采用三个规格为60 cm×40 cm×30 cm枕头对患者不同部位进行衬垫;对照组采用传统脊椎手术托架《型号JT-2》。比较两组患者体位摆放前后血压、心率变化情况、患者舒适度及患者皮肤受压状态;了解巡回护士、手术医生对改良体位垫应用效果的评价。结果体位摆放前后,两组患者血压和心率变化情况的组内比较,实验组无统计学意义(p>0.05),对照组具有统计学意义(p<0.05)。两组患者血压和心率变化情况的组间比较,差异具有统计学意义(均p<0.05)。实验组患者手术前后生命体征较对照组平稳,皮肤并发症发生率明显少于对照组,实验组患者的舒适度高于对照组;医护人员对改良体位垫的应用效果评价明显高于对照组,两组比较均p<0.05,差异具有统计学意义。结论体位垫在椎体形成术中,保证了患者手术前后生命体征的平稳,提高了患者的舒适度,减少体位并发症的发生,从而达到保护患者安全的目的,值得临床推广应用。
Objective To observe the effectiveness of posture pads in vertebral body formation surgery. Methods This study consisted of 60 patients who had vertebral body formation surgery they were equally divided into two groups in random. Patients in the experimental group used three specifications for 60 cm x 40 cm ~ 30 cm pil- low to patients with different parts of liner; while patients in the control group uses the traditional spinal surgery bracket named the model JT - 2. The change of blood pressure and heart rate before and after the settlement of the sur- gical position and patients comfortable rate and patients＇ skin compression were compared between the two groups. E- valuations of anesthetists, nurses and surgeons on these two groups were compared. Results The change of blood pressure and heart rate before and after the position placement was not significant within the experimental group （all p 〉 0.05 ） and was significant within the control group（ all p 〈 0. 05 ）. The change of blood pressure and heart rate was statistically significant between the two groups （all p 〈0. 05）. The incidence of skin complications were significantly lower in the experimental group than those in the control group. The patients comfortable rate higher than those in the control group; Medical personnel to improved position pad application effect evaluation is obviously higher than that of the control group（all p 〈 0. 05 ）. Conclusion The improved posture pad can ensure the patients with stable vital signs before and after the operation and reduce the incidence of complications in the vertebral body formation surgery, so it can ensure patients＇ safety and is worthy to apply in clinical practice.
