摘要
目的:观察早期肠内营养支持治疗对食管癌术后患者的临床效果。方法:将72例食管癌术后患者随机分为肠内营养(EN)组和肠外营养(PN)组,术后早期分别给予肠内及肠外营养支持治疗,观察比较两组术后营养状况、免疫功能及术后胃肠功能恢复时间、并发症及住院时间情况。结果:两组患者经营养治疗后白蛋白、前白蛋白及转铁蛋白等营养指标均较术前有明显改善(P<0.05),但两组间无明显差异(P>0.05);治疗后PN组BUN水平明显高于EN组(P<0.05);EN组患者术后第8天免疫学指标包括CD3+,CD4+,CD4+/CD8+比值及IgA、IgG、IgM均较术前显著改善(P<0.05),两组比较差异有统计学意义(P<0.05);EN组术后排气时间及住院时间均较PN组明显缩短(P<0.05)。结论:早期营养支持均能明显改善食管癌患者术后的营养状况,而EN在降低高分解代谢状态,提高机体免疫力、缩短胃肠功能恢复时间及住院日方面明显优于PN。
Objective:To observe the effects of early postoperative enteral nutrition in patients with esophageal cancer.Methods:72 patients with esophageal cancer were randomly divided into enteral nutrition(EN) and parenteral nutrition(PN) group,were given early enteral nutrition and parenteral nutrition support respectively.The nutritional status,immune function and postoperative gastrointestinal function recovery time,complications and hospitalization time were compared.Results:The levels of ALB,PA and TFN were improved significantly after treatment in both groups(P0.05),but no difference between the two groups(P0.05);But the BUN levels were significantly higher in PN group(P0.05);On the eighth day.The immunological indices including CD3+,CD4+,CD4+/CD8+ ratio and IgA,IgG,IgM improved significantly compared with the preoperative in EN group(P0.05),there wre significant difference between two groups(P0.05);The postoperative exhaust time and hospital stay were significantly shorter in EN group(P0.05).Conclusion:The early nutritional support can improve the nutritional status,and EN can improve the high catabolic and immunity status,reduce the gastrointestinal function recovery and hospitalization time.
