摘要
目的探讨新疆汉族、哈萨克族2型糖尿病(T2DM)脂代谢紊乱及胰岛素抵抗的差异及特点。方法选取2010年1月～2012年5月就诊的T2DM患者,哈萨克族100例,汉族102例;对照组,汉族53例,哈萨克族100例。检测空腹血糖(FPG)、血脂、糖化血红蛋白(HbA1c)、空腹胰岛素(FIns),计算胰岛素抵抗指数(HOMA-IR)及胰岛素分泌功能指数(HOMA-β),比较各组之间的差异。结果哈萨克族DM组体质量指数(BMI)、腰臀比(WHR)均高于其他各组(P<0.05);汉族、哈萨克族DM组TG、TC均高于各对照组(P<0.05),汉族DM组HDL-C低于其他各组,而其LDL-C高于其他各组(P<0.05),哈萨克族DM组FIns、HOMA-IR值明显高于其他各组(P<0.05);哈萨克族DM组HOMA-β高于汉族DM组(P<0.05)。结论汉族和哈萨克族DM患者均存在不同程度的脂代谢紊乱,但汉族DM患者具有明显的高LDL-C和低HDL-C;哈萨克族DM患者以胰岛素抵抗为主,而汉族DM患者以胰岛素分泌缺陷为主。
[Objective] To investigate the difference and characteristics in T2DM of Han people and Kazakh lived in Xinjiang. [Methods] Totally 202 T2DM patients （Kazakh 100, Han 102） who received medical treatment in our hospital from January 2010 to May 2012 were included in this study. Other 153 people （Kazakh 100, Han 53） were also selected as normal control. Fasting plasma glucose （FPG）, plasma lipid, HbAlc, fasting insulin （Fins） were measured, and HOMA-IR and HOMA-β were calculated. [Results] The BMI and WHR are highest in Kazakh T2DM group （P〈0.05）. The TG and TC are higher in the two T2DM groups than in normal control groups （P 〈0.05）. In Han T2DM group, there are lower HDL-C and higher LDL-C than other groups （P〈0.05）. The Fins and HOMA-IR are higher in Kazakh T2DM than in other groups （P〈0.05）. The HOMA-βof Kazakh T2DM is higher than Hart T2DM （P〈0.05）. [Conclusion] There is different degree of lipid disorder in both T2DM of Han and Kazakh. But in Han people, the increased LDL-C and decreased HDL-C are more significantly. The insulin resistance is the main eharacter in Kazakh T2DM. And the insulin defection is outstanding in Han T2DM.
