控制性低中心静脉压(LVCP)在肝叶切除手术中的应用 被引量:6

摘要 目的:研究控制性低中心静脉压对肝脏手术病人血流动力学、术中出血及肾功能的影响。方法:56例肝叶切除手术患者随机分成低中心静脉压组(A)组和对照组(B)组。A组从麻醉诱导后到肝实质横断分离完成时,液体输注速度严格控制在7 mL/h或1 mL/kg.min左右,将CVP维持<5 cmH2O。肝叶切除和止血完成后,以林格氏液和羟乙基淀粉补充液体缺失,恢复正常血流动力学。B组采用传统液体管理,维持CVP>10 cmH2O。比较两组患者术前,切肝前,切肝后和术后血流动力学变化、出血量以及肾功能变化。结果:两组患者围术期血流动力学变化影响差异不显著(P>0.05),LCVP组在肝叶切除术术中出血量、输血量均少于对照组(P<0.05),术后24 h肾功能变化无明显差异(P>0.05)。结论:控制性低中心静脉压可明显减少肝叶切除术中的出血量、输血量,对患者血流动力学和肾功能无明显影响,有利于患者术后康复。
