摘要
目的:探讨鼻内镜下手术治疗真菌性上颌窦炎的疗效及其影响因素。方法:2007年3月—2011年3月行鼻内镜下手术的真菌性上颌窦炎患者142例;采用1997年海口标准评估疗效,以Logistic多因素回归模型和单因素分析方法分析导致手术无效和术后复发的原因。结果:142例患者中,28例无效,114例有效;19例术后复发,123例未复发。年龄、病程、窦腔病变数、鼻腔手术史、病变窦腔内骨质破坏、糖尿病史均是影响手术效果和预后的独立因素;此外,影响手术效果的因素还包括干酪样分泌物,而2次手术、抽烟史和术后并发症。结论:鼻内镜下手术治疗真菌性上颌窦炎的疗效较好,但影响因素较多,应根据患者的具体病情制定相应的方案。
Objective：To explore the short-term and long-term clinical effect of endoscopic sinus surgery on fungal maxillary sinusitis and the related factors.Methods：A total of 142 cases of fungal maxillary sinusitis undergoing endoscopic sinus surgery from March 2007 to March 2011 were enrolled.The general information,conventional perioperative indicators and postoperative complications were retrospectively analyzed.The Logistic regression model was employed to analyze the influence factors.Results： In this study,28 cases were ineffective and 114 cases were effective;19 cases recurred and 123 patients were without recurrence.The independent factors for both surgery invalidity and postoperative recurrence included age,duration,number of sinus lesions,history of nasal surgery,whether the lesions of the sinus bone being destructed and history of diabetes.In addition,the independent factors also included cheese-like discharge for surgery invalidity;and second surgery,history of smoking,drinking history and postoperative complications were independent factors for recurrence.Conclusion： It is effective to treat fungal maxillary sinusitis with endoscopic sinus surgery,but there were many independent factors related with the efficacy.
