摘要
目的探讨二维、三维超声在诊断胎儿颜面部畸形的临床价值。方法采用表面成像或透明成像方法,对产前二维超声系统检查中发现或怀疑胎儿颜面部畸形的22例进行三维超声成像检查,将二维与三维超声的诊断结果与胎儿产后或引产后的检查结果进行对比分析。结果二维超声诊断的颜面部畸形22例,与产后结果相符15例,符合率68.2%,三维超声诊断与产后结果相符者20例,符合率90.9%,二维与三维超声诊断准确率比较差异有统计学意义(P<0.05)。结论三维超声可清晰、直观、全面地显示颜面结构的外形及比邻关系,可补充二维超声的不足,可进一步提高颜面部畸形的检出率。
Objective To discuss the clinical value of two and three-dimensional ultrasound in the diagnosis of fetal facial abnormalities. Methods Twenty-one pregnant women suspected or discovered as fetal facial abnormalities were examined with systemic prenatal two-dimensional ultrasound (2D US) and 3D US scan. Surface and transparent mode was used to reconstruct 3D images, which were compared with the results of post partum or after induction of labor. Results There were twenty-two facial abnormalities. Two-dimensional ultrasound diagnosis was consistent with postnatal diagnosis in fifteen cases,compliance rate of 68.2%. Three-dimensional ultrasound diagnosis was consistent with postnatal diagnosis in twenty cases,compliance rate of 90.9%, 3D US was significantly better than 2D US (P〈0. 05). Conclusion Three-dimensional ultrasound can more clearly,intuitively and exactly display the structures of the fetals' faces,which is very important supplement to two-dimensional ultrasound and raise greatly the detection rate of fetals'facial malfrmation.
