髓腔穿通病例的临床研究

Prospective study on clinical cases of perforation in pulp cavity

摘要 目的观察富士玻璃离子修复髓腔穿通的近期临床疗效。方法随机选取60例髓腔穿通的病例,清理髓腔和根管,髓腔内和根管内封入氢氧化钙类药物(Vitapex),1周后,常规根管充填,髓室底穿孔处用富士玻璃离子修复,最后用银汞或树脂修复。拍摄术前、术中、术后X线片。术后0.5、1、2年后随访,根据临床症状和X线片观察疗效。结果医源性穿孔和龋源性穿孔之间,疗效无明显差异(P>0.05);穿孔直径小于2 mm者及2~3 mm者,其疗效明显优于3 mm以上者(P<0.01)。结论富士玻璃离子具有良好的生物相容性,边缘密封好,微渗漏小,可以用于髓腔穿通的修补,但它的远期疗效尚需进一步观察。 Objective To observe the short-term clinical effects on repairing the perforation in pulp cavity with Fuji glass ionomer.Methods 60 molars perforated in pulp cavity caused by accident or caries were selected randomly and repaired with calcium hydroxide and Fuji glass ionomer.The bacteria from infected root canal and the perforation site were removed and medicated with calcium hydroxide for 1 week.The root canals were objurgated conventionally with a canal sealer and gutta-percha.Fuji glass ionomer was placed in the perforation site without pressure and the tooth was filled with amalgam or resin.Preoperative,operative and postoperative radiographs of each tooth were taken.The clinical follow-up at 6 months,1 and 2 years was evaluated according to clinical symptoms and X-ray examinations.Results It showed no significant difference(P0.05) between Iatrogenic perforation and the carious perforation,but The effective rate to the cases of perforation in pulp cavity(diameter2mm and diameter 2~3mm) were better than those of diameter3mm(P0.01).Conclusions Being better Biological compatibility,Edge sealing and smaller Micro seepage,Fuji glass ionome appears to be a valid option for repairing the perforation in pulp cavity but the long-term outcome studies should be performed.
