摘要
目的探讨术前肌肉注射甲氨蝶呤（MTX）及口服米非司酮联合腹腔镜保守性手术治疗未破裂异位妊娠的临床价值。方法回顾性分析2011年1月至2012年4月收治于郑州大学第一附属医院的生育期女性要求保留生育功能的40例异位妊娠患者的临床资料，分为治疗组和对照组，每组20例，治疗组给予术前应用MTX及米非司酮联合腹腔镜保守性手术，对照组给予单纯腹腔镜保守性手术，对比分析两组的术中出血量、手术时间、术后第4天血人绒毛膜促性腺激素（HCG）下降率、术后宫内妊娠率。结果与对照组相比，治疗组的术中出血量和手术时间均显著降低，术后第4天血人绒毛膜促性腺激素（HCG）下降率和术后宫内妊娠率显著提高，两组比较差异有统计学意义（P〈0．05）。结论术前应用MTX及米非司酮联合腹腔镜保守性手术治疗未破裂型异位妊娠疗效好、恢复快，有重要·临床应用价值，可作为术前常规用药。
Objective To investigate the clinical value of preoperative intramuscular injection of methotrexate（MTX） and oral taken of mifepristone combined with laparoscopic conservative sur- gery on unruptured ectopic pregnancy. Methods The clinical data of 40 ectopie pregnancy cases who require reproductive function treated in the first affiliated hospital of Zhengzhou university from 2011 January. to 2012 April were＇ retrospectively analzed. And they were divided into the therapeutic group（20 cases） and Control group（20 cases）. The patients in the therapeutic group were treated with preoperative application of MTX and mifepristone combined with laparoscopic conservative surgery, the patients in control group were treated with laparoscopic conservative surgery only. The amount of intraoperative bleeding, operation time, decreasing rate of serum human chorionic gonadotropin （HCG） at the postoperative fourth day and incidence of postoperative intrauterine pregnancy in the two groups were compared. Results Compared with the control group, the amount of intraoperative bleeding and operation time in the therapeutic group decreased significantly, decreasing rate of serum HCG at the postoperative fourth day and incidence of postoperative intrauterine pregnancy increased significantly, the difference was significant（ P 〈0.05）. Conclusions Preoperative application of MTX and mifepristone combined with laparoscopic conservative operation in the treatment of unruptured ectopie pregnancy showed good curative effect and quick recovery, and it had important clinical value. It can be used as a routine preoperative medication.
