摘要
目的对临床常用的3种内固定方法用于髌骨骨折治疗中的临床疗效进行比较,寻求最佳的固定方法用于临床推广。方法选取我院2008年6月~2010年6月采用内固定方式治疗的髌骨骨折患者90例为研究对象,对其临床资料进行回顾性分析。按照固定方式的不同,将其分为3组,每组研究对象30例。使用钢丝粗线环扎术固定的为观察1组;镍钛-聚髌器内固定的为观察2组;使用改良张力带钢丝内固定术的为观察3组。随访16个月,对3组研究对象的临床疗效进行比较,并进行统计学分析。结果所有研究对象均获随访,3组研究对象的骨折愈合率分别为73.3%(22/30),93.3%(28/30),86.7%(26/30);观察2组优于其他两组,差异有统计学意义(P<0.05);在活动功能及患者满意率等数据的比较中,观察2组优于其他两组,差异有统计学意义(P<0.05)。结论在髌骨骨折的固定中镍钛-聚髌器内固定较钢丝粗线环扎术及改良张力带钢丝内固定术具有愈合率高,活动功能恢复好,患者满意率高等优势,适于临床推广使用。
Objective To seek the optimal fixation method for clinical promotion through comparing the clinical efficacy of three common internal fixation methods for patella fracture fixation. Methods The clinical data of 90 patients with patella fracture who received internal fixation treatment in our hospital from June 2008 to June 2010 were analyzed retrospectively. These patients were divided into 3 groups according to the fixation method, with 30 patients in each group. The patients using steel wire thick line cerclage for fixation were assigned to th~ observation group 1;the patients using nickel titanium-patellar concentrator for internal fixation were assigned to the observation group 2;the patients using modified tension band steel wire for internal fixation were assigned to the observation group 3. After 16 months of follow-up,the clinical efficacy of the 3 groups was compared and analyzed statistically. Results All the patients were followed up. The fracture healing rates of the 3 groups were 73.3%(22/30),93.3%(28/30) and 86.7%(26/30) respectively;the observation group 2 was superior to the other 2 groups,with statistically significant difference(P 〈 0.05). The observation group 2 was superior to the other 2 groups in the activity function and patient satisfaction rate,with statistically significant difference(P 〈 0.05). Conclusion In the fixation of patella facture,the nickel titanium-patellar concentrator internal fixation has higher healing rate,better activity function recovery and higher patient satisfaction rate than the steel wire thick line cerclage and the modified tension band steel wire internal fixation,thereby worthy of clinical promotion and application.
