摘要
目的探讨助理护士管理模式在医院感染管理中的作用,以提高护理质量,降低医院感染的风险。方法2011年1月起在ICU、新生儿室、供应室、内镜室4个医院感染重点部门设立助理护士,取代护工,承担基础护理和清洁消毒工作;院感科分别于2010年12月和2012年1月对以上科室的护工或助理护士进行手卫生依从性和医院感染基础知识的调查,并比较实施助理护士管理模式前后的差异。结果实施助理护士管理模式前后平均年龄分别是55岁和20岁,实施助理护士管理模式前文化程度以小学为主,实施助理护士管理模式后均为护理学中专;实施助理护士管理模式前后手卫生依从性分别为34.3%、69.2%,医院感染基础知识合格率为31.6%、89.5%;实施助理护士管理模式前后手卫生依从性和医院感染基础知识合格率差异均有统计学意义(P<0.05)。结论在医院感染重点部门实施助理护士的管理模式,可提高护理质量,降低医院感染的风险,并为中专护士就业难找到了一条出路,值得推广。
OBJECTIVE To explore the role of assistant nurses in the process of nosocomial infection control so as to improve the nursing quality and reduce the risk of nosocomial infections.METHODS From Jan.2011,the assistant nurses were employed in ICU,neonatal room,supply room,and endoscopy room,those nurses replaced the nursing worker and took charge of the primary care and cleaning and disinfection.The compliance of hand hygiene and the knowledge related with infection control of the nursing workers or the assistant nurses were investigated by the infection control branch in Dec 2010 and Jan 2012,the differences were compared before and after the interventions.RESULTS The average age were 55 years old and 20 years old before and after the intervention,education level was mainly primary school before the intervention,nursing secondary technical school after the intervention.The compliance rates of the hand hygiene were respectively 34.3% and 69.2% before and after the intervention,the qualified rates of the related knowledge were 31.6% and 89.5%,respectively.The differences in the compliance of hand hygiene and the related knowledge between before and after the interventions were statistically significant（P0.05）.CONCLUSION The assistant nurses play an important role in the process of infection control in the key departments of nosocomial infection,and we find it is a good way for the nurses of secondary technical school to be employed.
