摘要
目的对比分析保留乳头乳晕复合体的乳腺癌改良根治术与传统乳腺癌改良根治术的疗效。方法将2005年2月-2012年2月收治的80例早期(Ⅰ、Ⅱa期)乳腺癌患者随机分为保留组30例和传统组50例,保留组采用保留乳头乳晕复合体的乳腺癌改良根治术,传统组采用传统乳腺癌改良根治术,对比观察两组的术后效果。结果两组手术均成功,无严重并发症发生。术后随访保留组局部复发2例(6.67%),远处转移1例(3.33%),无瘤生存期为(15.0±3.4)个月;传统组分别为3例(6.00%)、2例(4.00%)及(14.0±4.3)个月;两组比较,P均>0.05。保留组手术美容学评价为优18例(60.00%),良12例(40.00%),差0例;对照组分别为20例(40.00%)、22例(44.00%)、8例(16.00%),两组优良率比较,P<0.05。结论保留乳头乳晕复合体的乳腺癌改良根治术与传统乳腺癌改良根治术均可取得良好效果,但保留乳头乳晕复合体的乳腺癌改良根治术美容效果好。
Objective To compare the efficacy of reserving nipple-areola complex modified radical mastectomy and traditional modified radical mastectomy. Methods From February 2005 to February 2012,80 patients were treated early（ Ⅰ ,Ⅱ a period） , breast cancer patients were randomly divided into retention group （30 cases）and traditional group（50 cases） ,in retention group, the nipple-areola complex was reserved with breast cancer modified radical mastectomy, and traditional group with traditional breast modified radical mastectomy, postoperative results were compared and analyzed. Results The surgery was successful in both groups, with no serious complications. In the retention group, postoperative follow-up of local recurrence in 2 cases （6.67%）,distant metastasis cases（3.33% ）, disease-free survival（ 15.0-＋ 3.4） months;while the tradition group were 3 cases （ 6.00% ） , two cases （ 4.00% ） , and （ 14.0 -＋ 4.3 ） months, respectively. The difference were not significant （ P 〉 O. 05 ）. In reten- tion group, cosmetic surgery evaluation was excellent in 18 cases （60.00%）, good in 12 cases （40.00%）, and poor in 0 case;in control group were 20 cases（40.00% ） ,22 cases（44.00% ） and 8 cases（ 16.00% ） ,respectively. The difference were significant （P 〈 0.05 ）. Conclusion Keeping the nipple areola complex modified radical mastectomy and traditional modified radical mas- tectomy can achieve good results, but keeping the nipple-areola complex modified radical masteetomy was better with cosmetic re- suits.
