摘要
目的探讨影响喉癌患者手术切除后5年生存率因素及不同术式生存质量的比较。方法选择2005年1月—2008年1月首次行手术切除治疗的喉癌患者80例的临床资料进行回顾性分析,随访术后5年生存率并分析相关影响因素;采用华盛顿大学生存质量问卷(UW-QOL)评定不同术式喉癌患者的生存质量。结果 80例喉癌患者5年总生存率为66.3%(53/80)。单因素分析显示:不同年龄、原发部位、分化程度、切缘状态、临床分期、T分期及N分期喉癌患者5年生存率比较差异有统计学意义(P<0.05)。喉部分切除术组患者UW-QOL量表中总分、疼痛、外貌、娱乐、语言交流等项目评分与喉全部切除术组比较差异有统计学意义(P<0.05)。结论影响喉癌患者术后生存率的因素主要有年龄、原发部位、分化程度、切缘状态、临床分期、T分期及N分期,不同治疗方案并不影响喉癌患者的生存率,但喉部分切除术能够提高患者术后的生活质量。
Objective To explore the impact of the 5-year survival factors and different procedures of quality of life in patients with laryngeal cancer after surgical resection. Methods From January 2005 to January 2008 for the first time laryngeal surgical resection treatment of 80 patients with clinical data were retrospectively analyzed, follow-upthe 5-year survival of the postoperative and analysis of related factors; use the University of Washington quality of life questionnaire （UW-QOL） assess- ment of the quality of life of the different surgical patients with laryngeal cancer. Results 80 cases of patients with laryngeal cancer 5-year overall survival rate was 66.3% （53/80）. Univariate analysis showed that： different ages, the original site of differentiation, margin status, clinical stage, T stage and N stage patients with laryngeal cancer 5-year survival difference were statistically significant （ P 〈 0.05）. Partial laryngectomy patients UW-QOL scale in total, pain, appearance, entertainment, language exchange project score and laryngeal resection group, the difference was statistically significant （ P 〈 0.05 ）. Con- clusion Factors affecting postoperative survival rate of patients with laryngeal cancer age, primary site, degree of differentia- tion, margin status, clinical stage, T stage and N stage, the different treatment options does not affect the survival rate of pa- tients with laryngeal cancer, however, partial laryngectomy can improve the quality of life of patients.
