摘要
目的观察术前同期放化疗对桂西壮族直肠癌患者的治疗效果和预后的影响。方法选取80例桂西地区壮族直肠癌患者为观察对象,其中行术前同期放化疗48例患者为术前同期放化疗组,行术前放疗32例患者为术前放疗组,回顾性分析两组患者的临床资料,比较两组患者的治疗效果、保肛率、降期率以及生存率。结果术前同期放化疗组治疗总缓解率为64.58%,总有效率为81.25%,均高于术前放疗组的40.63%和59.38%,比较差异均有统计学意义(χ2值分别为4.45和4.61,P<0.05)。术前同期放化疗组患者保肛率为68.75%,术前放疗组患者保肛率为62.5%,两组比较无统计学意义(χ2=0.34,P>0.05)。术前同期放化疗组降期率为54.17%,术前放疗组为53.14%,比较差异无统计学意义(χ2=0.01,P>0.05)。术前同期放化疗组患者术后3年生存率为66.56%,高于术前放疗组的38.31%,比较差异有统计学意义(P<0.05)。结论术前同期放化疗或术前放疗均有助于提高桂西壮族直肠癌患者的保肛率和降期率,而术前同期放化疗还能明显提高患者的肿瘤缓解率、治疗有效率和生存率,获得满意的治疗效果和预后。
Objective To observe the curative effect and prognosis of the preoperative concomitant chemoradio- therapy for Zhuang patients with colorectal cancer in the west region of Guangxi Zhuang Autonomy. Methods 80 ca- ses of Zhuang patients with colorectal cancer in the west of Guangxi Zhuang Autonomous Region were selected as the observation objects,of which 48 cases of patients were the preoperative concomitant chemoradiotherapy group and 32 cases of patients were the preoperative radiotherapy group. The clinical data about the treatment effects, the rate of a- nal preservation, the rate of descent stage and the survival rate of the two groups were compared and retrospectively analyzed. Results The total remission rate and the total effective rate of the preoperative concomitant chemoradio-therapy group were 64.58 and 81.25 % respectively, significantly higher than those of the preoperative radiotherapy group--40.63; and 59.38%. So the difference was statistically significant （X2 =4.45,X2 =4.61, P d0.05）. The anal preservation rate of the preoperative concomitant ehemoradiotherapy group was 68.75%, which was not signifi- cantly different from that of the preoperative radiotherapy group-62.5% （X2 =0.34, P〉0.05）. The rate of descent stage of the preoperative concomitant chemoradiotherapy group was 54.17 % ,while that of the preoperative ra- diotherapy group was 53.14 %. So the difference was also not statistically significant （X2 = 0.01, P〉 0.05）. Three years after the surgery, the survival rate of the preoperative concomitant chemoradiotherapy group was 66.56%, which was significantly higher than that of the preoperative radiotherapy group-38.31% （ P〈0.05）. Conclusion The preoperative concomitant chemoradiotherapy and the preoperative radiotherapy can help to improve the anal preservation rate and the rate of descent stage of the patients with coloreetal cancer in the west of Guangxi Zhuang Autonomous Region. But the preoperative concomitant chemoradiotherapy can also obviously improve the remission rate,the effective rate of treatment and the survival rate, which can help achieve satisfactory treatment effect and prognosis.
